耳穴揿针联合足三里穴位注射盐酸甲氧氯普胺改善妇科恶性肿瘤化疗相关性恶心呕吐的疗效观察

摘　　要：目的观察耳穴揿针联合足三里穴位注射盐酸甲氧氯普胺改善妇科恶性肿瘤化疗相关性恶心呕吐(CCINV)的临床疗效,评估其对患者生活质量的改善作用。方法选取50例妇科恶性肿瘤患者,根据随机数字表法分为对照组与研究组,每组25例。对照组患者应用托烷司琼止吐,研究组在对照组基础上应用耳穴揿针联合足三里穴位注射盐酸甲氧氯普胺。干预后评估两组患者呕吐恶心状况,并应用恶心呕吐生活功能量表评估患者生活质量。结果对照组患者治疗有效率为64.0%,研究组患者治疗有效率为88.0%,研究组治疗有效率高于对照组,差异具有统计学意义(P<0.05)。研究组恶心呕吐评估评分低于对照组,首次呕吐平均时间长于对照组,呕吐持续平均时间短于对照组,差异具有统计学意义(P<0.05)。结论应用耳穴揿针联合足三里穴位注射盐酸甲氧氯普胺能够显著改善妇科恶性肿瘤患者化疗后恶心呕吐状况,缩短呕吐时间,缓解患者对化疗的恐惧感,从而提高化疗的依从性。Objective To observe the clinical effect of ear acupuncture combined with metoclopramide hydrochloride injection at Zusanli acupoint in improving chemotherapy-associated nausea and vomiting(CCINV)of gynecological malignant tumor,and to evaluate its effect on patients'quality of life.Methods 50 patients with gynecological malignant tumor were selected and divided into control group and study group according to random number table method,with 25 cases in each group.The control group was treated with troisetron to stop vomiting,and the study group was treated with metoclopramide hydrochloride injection at the auricular point combined with Zusanli point on the basis of the control group.After intervention,the vomiting and nausea status of the two groups were evaluated,and the quality of life of the patients was assessed by the nausea and vomiting life function scale.Results The treatment effective rate was 64.0%in the control group and 88.0%in the study group.The treatment effective rate in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).The nausea and vomiting evaluation score of the study group was lower than that of the control group,the average time of first vomiting was longer than that of the control group,and the average duration of vomiting was shorter than that of the control group,with statistical significance(P<0.05).Conclusion The application of ear point acupuncture combined with Zusanli acupoint injection of metoclopramide hydrochloride can significantly improve the nausea and vomiting of patients with gynecological malignant tumor after chemotherapy,shorten the vomiting time,relieve the patients'fear of chemotherapy,and improve the compliance of chemotherapy.
