胰岛素联合饮食指导对妊娠期糖尿病血糖调控及母婴结局的影响

摘　　要：目的探究妊娠期糖尿病患者治疗时采用胰岛素联合饮食指导对其血糖指标及母婴结局的影响。方法选取46例妊娠期糖尿病患者,按照抽签法分为对照组与观察组,各23例。对照组接受胰岛素治疗,观察组在对照组基础上加入饮食指导。比较两组治疗前后血糖与糖化血红蛋白水平及不良妊娠结局情况。结果治疗前,两组空腹血糖、餐后2h血糖、糖化血红蛋白水平比较,差异无统计学意义(P>0.05);治疗后,两组空腹血糖、餐后2h血糖、糖化血红蛋白水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组不良妊娠结局发生率为21.74%,低于对照组的60.87%,差异有统计学意义(P<0.05)。结论对妊娠期糖尿病患者应用胰岛素联合饮食指导效果良好,可有效调节患者的血糖水平,改善母婴结局,值得临床推广。Objective To explore the effects of insulin combined with dietary guidance on blood glucose index and maternal and infant outcomes in patients with gestational diabetes mellitus.Methods 46 patients with gestational diabetes mellitus were selected and divided into control group and observation group,with 23 cases in each group.The control group received insulin treatment,and the observation group was supplemented with dietary guidance.The levels of blood glucose and glycosylated hemoglobin and adverse pregnancy outcomes before and after treatment were compared between the two groups.Results Before treatment,there was no significant difference in fasting blood glucose,2 h postprandial blood glucose and glycated hemoglobin levels between the two groups(P>0.05).After treatment,the levels of fasting blood glucose,2 h postprandial blood glucose and HBAlc in both groups were lower than before treatment,and the observation group was better than the control group,with statistical significance(P<0.05).The incidence of adverse pregnancy outcome in the observation group was 21.74%,lower than that in the control group(60.87%),and the dfference was statistically significant(P<0.05).Conclusion Insulin combined dietary guidance for gestational diabetes patients has good effect,can effectively regulate the blood sugar level of patients,improve maternal and infant outcomes,and is worthy of clinical promotion.
