负压封闭引流技术联合临床护理路径模式全程护理在伴软组织损伤老年烧伤患者中的应用研究

摘　　要：目的观察负压封闭引流(VSD)技术联合临床护理路径(CNP)模式的全程护理在伴软组织损伤老年烧伤患者中的应用效果。方法选取2018年7月—2021年6月绵阳市中心医院94例伴软组织损伤老年烧伤患者,按照入院时间分为2组,每组各47例。对照组实施基础护理干预,联合组在对照组基础上实施VSD技术联合CNP模式的全程护理干预。比较2组细菌感染率、创面愈合时间、住院时间、干预前、干预3 d后、干预7 d后视觉模拟评分法(VAS)评分、干预前后简明健康状况调查量表(SF-36)评分、护理满意度。结果联合组细菌感染率6.38%较对照组21.28%低,创面愈合时间、住院时间较对照组短(P<0.05);干预3 d后、干预7 d后联合组VAS评分较对照组低(P<0.05);联合组身体疼痛、生理功能、精神健康、一般健康状况评分较对照组高(P<0.05);联合组护理满意度95.74%较对照组78.72%高(P<0.05)。结论VSD技术联合CNP模式的全程护理干预应用于伴软组织损伤老年烧伤患者,可减少细菌感染,减轻术后疼痛,加快创面愈合、患者康复,改善患者生活质量,提高护理满意度。Objective To observe the application effect of vacuum sealing drainage(VSD)technology combined with clinical nursing pathway(CNP)mode of whole-course nursing in elderly burn patients with soft tissue injury.Methods A total of 94 elderly burn patients with soft tissue injury in The Mianyang central hospital from July 2018 to June 2021 were selected and grouped according to the time of admission,with 47 cases in each group.The control group was given basic nursing intervention,and the combined group was given the whole-course nursing intervention of VSD technology combined with CNP model on the basis of the control group.The bacterial infection rate,wound healing time,hospital stay,and Visual Analogue Scale(VAS)scores before and after the intervention,3 days and 7 days after the intervention were compared between the two groups.Comparison of scores and nursing satisfaction after intervention.Results The bacterial infection rate in the combination group was 6.38%lower than that in the control group(21.28%),and the wound healing time and hospital stay were shorter than those in the control group(P<0.05).After 3 days of intervention and after 7 days of intervention,the VAS score of the combined group was lower than that of the control group(P<0.05).After the intervention,the scores of physical pain,physiological function,mental health,and health changes in the combined group were higher than those of the control group(P<0.05).After the intervention,the nursing satisfaction of the combined group was 95.74%higher than that of the control group(78.72%)(P<0.05).Conclusion The whole-course nursing intervention of VSD technology combined with CNP model is applied to elderly burn patients with soft tissue injury,which can reduce bacterial infection,relieve postoperative pain,accelerate wound healing and patient recovery,improve patients'quality of life,and improve their nursing satisfaction.
