妇产科Ⅱ类切口手术患者围术期预防性应用抗菌药物的合理性分析

摘　　要：目的探讨对妇产科Ⅱ类切口手术患者围术期预防性应用抗菌药物的合理性。方法选取150例妇产科Ⅱ类切口手术患者,对其预防性应用抗生素的信息进行分析,统计预防性应用抗菌药物品种选择、术前用药的时机及术后用药的维持时间。结果150例患者预防性应用抗菌药物以单用头孢西丁为主,占82.67%,其次为头孢西丁+吗啉硝唑、头孢尼西。患者预防用药时机主要在术前0.5~1h,但也有部分病例在术前1~2h用药,或在术后预防用药。患者预防性应用抗生素时,预防用药疗程≤24h的占52.67%,预防用药疗程>24h且≤48h的占18.67%。结论在妇产科Ⅱ类切口手术患者中预防性应用抗生素的情况较为常见,但围术期预防性应用抗菌药物在药物品种的选择、预防给药时机及疗程方面仍存在不合理现象,应加强药学干预。Objective To explore the rationality of prophylactic use of antibiotics in the perioperative period in patients with Class Ⅱ incision surgery in obstetrics and gynecology.Methods 150 patients with Class Ⅱ incision in obstetrics and gynecology were selected to analyze the information of prophylactic antibiotic use.The selection of prophylactic antibiotics,the timing of preoperative administration and the maintenance time of postoperative administration were analyzed.Results Among 150 patients,cefoxitin alone was the main prophylactic antibiotic,accounting for 82.67%,followed by cefoxitin+morinidazole and cefonicil.The time for prophylactic drug use in patients was mainly 0.5 to 1 h before surgery,but some cases were also used 1 to 2 h before surgery or after surgery.When the prophylactic antibiotics were used,the duration of prophylactic drugs≤24 h accounted for 52.67%,and the duration of prophylactic drugs>24 h and≤48 h accounted for 18.67%.Conclusion Prophylactic use of antibiotics is common in patients with Class Ⅱ incision surgery in obstetrics and gynecology,but the prophylactic use of antibiotics in perioperative period still has unreasonable phenomena in the selection of drug varieties,the timing of prophylactic administration and the course of treatment,so pharmaceutical intervention should be strengthened.
