摘　　要：目的观察针对性叙事护理干预对肿瘤恶化转折期患者的干预效果。方法选取无锡市锡山人民医院2021年1月—2022年4月收治的30例晚期恶性肿瘤患者作为研究对象,患者均于纳入研究的前2周内出现肿瘤复发和转移,且无有效的临床治疗方法。将2021年6月1日前入院的15例患者设为对照组,此后入院的15例患者设为实验组。对照组采用肿瘤内科常规护理干预,实验组采用针对性叙事护理干预,比较2组患者干预前后生存质量(社会功能、情感职能、精神健康维度)评分、疼痛评分和心理痛苦指数评分。结果干预前,2组患者生存质量评分、疼痛评分和心理痛苦指数评分比较,差异均无统计学意义(P>0.05)。干预后,实验组患者社会功能、情感职能维度评分均高于对照组,但差异无统计学意义(P>0.05);干预后,实验组精神健康维度评分高于对照组,疼痛评分、心理痛苦指数评分低于对照组,差异有统计学意义(P<0.05)。结论将针对性叙事护理干预应用于肿瘤恶化转折期患者,可降低其疼痛程度和心理痛苦指数,提升其生命质量,稳定其精神状态,使其更平稳地过渡至临终阶段,且方法简单易行。Objective To observe the effect of narrative nursing intervention in patients with tumor deterioration in the transition stage.Methods Thirty patients with advanced malignant tumors admitted to Xishan People′s Hospital of Wuxi City from January 2021 to April 2022 were selected as study objects.All patients had tumor recurrence and metastasis within the first 2 weeks of inclusion,and there was no effective clinical treatment.A total of 15 patients admitted before June 1,2021 were selected as control group,and 15 patients admitted after June 1,2021 were selected as experimental group.The control group received routine nursing of oncology department of internal medicine,and the experimental group received targeted narrative nursing intervention.The scores of quality of survival(social function,emotional function,mental health dimension),pain score and psychological pain index score were compared between the two groups before and after intervention.Results Before intervention,there were no significant differences in quality of survival score,pain score and psychological pain index score between two groups(P>0.05).After intervention,the scores of social function and emotional function in the experimental group were higher than those in the control group,but there were no statistically significant difference(P>0.05).After intervention,the mental health dimension score of the experimental group was higher than that of the control group,and the pain score and psychological pain index score were lower than that of the control group(P<0.05).Conclusion The implementation of targeted narrative nursing intervention at the transition stage of advanced tumor deterioration can relieve the degree of pain,reduce psychological pain index of patients,improve the quality of life,stabilize mental state,and have important significance for a more stable transition to the terminal stage.Besides,it is a simple and easy method.
