针灸临床实践指南的发展

摘　　要：梳理了针灸指南的研制现状,发现在当前针灸指南的研制过程中,制定者不仅面临着证据困境,同时因未充分考虑针灸学特性,从而影响针灸指南对临床实践的指导作用。为解决以上问题,提出了针灸指南临床问题的确定方法、古今医家经验在指南中的转化应用方法,并针对推荐方案的共识过程提出了方法学建议。建议未来在发展针灸指南及针灸学科时,应重点关注:构建符合针灸学特性的“证据体系”、规范指南的共识过程以及建立适应针灸学科的科研成果推广体系等方面。This article reviews the status of acupuncture-moxibustion clinical practice guidelines(CPGs),and discovers that during the formulation of acupuncture-moxibustion CPGs,the developers not only face the evidence dilemma,but also pay less attention to the disciplinary characteristics of acupuncture-moxibustion,which affects the clinical guidance of acupuncturemoxibustion CPGs.To solve these problems,this article presents the determination methods of clinical questions in acupuncturemoxibustion CPGs,the transformation and application methods in guidelines of clinical experiences of ancient and modern acupuncturists,and the methodological suggestions related to the consensus process of forming the treatment protocols.In the future,the aspects which should be taken into account during the development of acupuncture-moxibustion CPGs and disciplines include the construction of the"evidence system"conformed to the characteristics of acupuncture-moxibustion,the standardization of the consensus process of CPGs and the establishment of a promotion system of research achievements conformed to the characteristics of acupuncture-moxibustion.
