摘 要:目的 采用有限元分析方法比较低模量碳纤维增强聚醚醚酮(CFR-PEEK)髓内钉与钛合金髓内钉固定胫骨中段骨折的效果。方法 采用钛合金和不同碳纤维含量CFR-PEEK(CF30、CF50、CF60)髓内钉固定胫骨干粉碎性骨折,分析皮质骨最大应力、骨折最大位移。结果 在轴向压缩200 N载荷条件下,髓内钉最大应力集中在胫骨近端区域,钛合金组最大应力为104.2 MPa,CF30组为32.7 MPa,CF50组为103.3 MPa,CF60组为103.1 MPa。在轴向压缩700 N载荷条件下,髓内钉最大应力也位于胫骨近端区域,钛合金组最大应力为294.2 MPa,CF30组为175.0 MPa, CF50组为143.6 MPa,CF60组为145.0 MPa。在轴向压缩200 N、700 N载荷条件下,钛合金组最大位移分别为1.07 mm和3.15 mm,CF30组分别为4.25 mm和8.17 mm,CF50组分别为2.02 mm和4.15 mm,CF60组分别为1.94mm和4.08 mm。结论 CF50、CF60髓内钉可安全应用于胫骨干粉碎性骨折,而CF30髓内钉不适用于胫骨干粉碎性骨折。CFR-PEEK材料具有一定的生物力学优势,值得进一步研究。Objective To compare low-modulus carbon fiber-reinforced polyether ether ketone(CFR-PEEK)intramedullary nails with titanium alloy intramedullary nails by finite-element analysis.Methods Titanium alloy(Ti6Al4V)and CF-PEEK(CF30,CF50,and CF60)intramedullary nails with different carbon fiber contents were used to fix comminuted tibial shaft fractures.The maximum stress of cortical bone and the maximum fracture displacement were analyzed.Results Under an axial compression load of 200 N,the maximum stress of the intramedullary nails was concentrated in the proximal region of the tibia.The maximum stress was 104.2 MPa in the titanium alloy group,32.7 MPa in the CF30 group,103.3 MPa in the CF50 group,and 103.1 MPa in the CF60 group.Under an axial compression load of 700 N,the maximum stress of the intramedullary nails was also located in the proximal region of the tibia.The maximum stress was 294.2 MPa in the titanium alloy group,175.0 MPa in the CF30 group,143.6 MPa in the CF50 group,and 145.0 MPa in the CF60 group.Under axial compression of 200 N and 700 N,the maximum displacement of the intramedullary nails was 1.07 and 3.15 mm in titanium alloy group,4.25 and 8.17 mm in the CF30 group,2.02 and 4.15 mm in the CF50 group,and 1.94 and 4.08 mm in the CF60 group,respectively.Conclusion CF50 and CF60 intramedullary nails can be safely applied to tibial shaft comminuted fractures,but CF30 intramedullary nails are not suitable for these fractures.Finite-element analysis shows that CFR-PEEK materials have certain biomechanical advantages and are worth further research.
