拇外翻微创截骨术后“8”字绷带固定对截骨端背侧移位影响的尸体研究

摘　　要：目的:探讨“8”字绷带外固定控制中西医结合拇外翻截骨术后截骨端背侧移位的效果。方法:采用9具自然解冻成人新鲜尸体标本的双下肢(18足)第一跖骨,通过中西医结合拇外翻微创截骨术并手法整复后,将每个标本的左足使用随机数字表法分配到对照组或实验组,右足则自动分到另一组。实验组行“8”字绷带外固定,对照组使用部分螺纹克氏针内固定,固定完成后,所有模型分别接受300、600、750 N的轴向载荷力并记录截骨端背侧移位数值。比较两种固定方式下前足受轴向压力时截骨端背侧移位的差异。结果:两种固定方式间背侧移位比较,差异无统计学意义(P>0.05)。结论:“8”字绷带外固定与部分螺纹克氏针内固定在控制中西医结合拇外翻截骨术后截骨端背侧移位方面有相近的固定效果,推荐在临床中使用。Objective:To investigate the effect of"Fige-8"bandage external fixation on controlling the dorsal displacement of osteotomy end after hallux valgus osteotomy.Methods:The first metatarsal bones of both lower limbs(18 feet)of 9 naturally thawed fresh adult cadavers were treated with integrated traditional Chinese and western medicine minimally invasive hallux valgus osteotomy and manual reduction.The left foot of each specimen was randomly assigned to the control group or the experimental group,and the right foot was automatically assigned to the other group.The experimental group was fixed with a"figue-8"bandage,and the control group was fixed with a partially threaded Kirschner wire.After fixation,all models were subjected to axial loading forces of 300,600 and 750 N,respectively,and the dorsal displacement of the osteotomy end was recorded.The difference of dorsal displacement of the osteotomy end under axial pressure of the forefoot was compared between the two fixation methods.Results:There was no significant difference in dorsal displacement between the two fixation methods(P>0.05).Conclusion:The"fige-8"bandage external fixation and partial thread Kirschner wire internal fixation have similar fixation effect in controlling the dorsal displacement of the osteotomy end,which is recommended for clinical use.
