倪海雯应用芪苓白头翁汤治疗脾虚湿热型原发肠道弥漫大B细胞淋巴瘤临证经验

摘　　要：总结倪海雯治疗原发肠道弥漫大B细胞淋巴瘤的经验。原发肠道弥漫大B细胞淋巴瘤病情凶险,预后差,临床表现缺乏特异性,治疗棘手。倪海雯以周仲瑛癌毒学说及病机十三条理论为指导,从“虚、毒、湿、热”复合病机论治,归纳本病核心病机为“脾虚癌毒,挟杂湿热”,并结合不同治疗阶段采用分期论治、病证结合的中西整合模式,在经典名方白头翁汤基础上结合癌毒病机加以创新,创立验方芪苓白头翁汤以健脾益气,燥湿清肠,标本兼治。临床随访观察发现,其可明显改善患者临床症状,减少复发,提高患者生活质量。To summarize NI Haiwen's experience in treating primary diffuse large B-cell lymphoma of the intestine.Primary intestinal diffuse large B-cell lymphoma is a dangerous disease with poor prognosis,lack of specific clinical manifestations and difficult treatment.Guided by ZHOU Zhongying's theory of cancer toxicity and 13 theories of pathogenesis,NI Haiwen summarized the core pathogenesis of the disease as"spleen deficiency cancer toxicity,mixed with dampness and heat"from the combination of pathogenesis of"deficiency,toxicity,dampness and heat".Combined with different treatment stages,the Chinese and western integration model of stage treatment,disease and syndrome combination was adopted.Based on the classic recipe Baitouweng Decoction(白头翁汤)and the pathogenesis of cancer,Qiling Baitouweng Decoction was established to strengthen spleen and Qi,dry dampness and clear intestine,and treat both symptoms and root causes.Clinical follow-up observations have found that this formula can significantly improve patients'clinical symptoms,reduce recurrence,and improve their quality of life.
