低分子肝素钙联合丙种球蛋白对小儿过敏性紫癜性肾炎患者的影响

摘　　要：目的:分析低分子肝素钙联合丙种球蛋白对小儿过敏性紫癜性肾炎患者的影响。方法:选取2021年4月-2022年4月湖北省第三人民医院阳逻院区收治的100例小儿过敏性紫癜性肾炎患儿,按照随机数表法将其分为两组,各50例。对照组采取常规治疗+低分子肝素钙治疗,观察组采用常规治疗+低分子肝素钙+丙种球蛋白治疗,对比两组临床治疗效果。结果:观察组总有效率明显较对照组高,差异有统计学意义(P<0.05);观察组治疗后CD4^(+)、CD4^(+)/CD8^(+)、干扰素-γ(IFN-γ)水平高于对照组,CD8^(+)、白细胞介素-4(IL-4)、纤维蛋白原(FIB)、部分凝血活酶时间(APTT)、凝血酶原时间(PT)、尿微量白蛋白(mALB)和β_(2)-微球蛋白(β_(2)-MG)水平低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:小儿过敏性紫癜性肾炎患者采取低分子肝素钙与丙种球蛋白联合治疗,能有效改善免疫功能、凝血功能,减轻机体炎症反应,且不良反应较少,安全性高,临床效果显著。Objective:To analyze the effect of Low Molecular Weight Heparin Calcium combined with Gamma Globulin on allergic purpura nephritis in children.Method:A total of 100 children with allergic purpura nephritis who admitted to Yangluo Hospital,the Third People's Hospital of Hubei Province from April 2021 to April 2022 were selected and divided into two groups according to random number table method,50 cases in each group.The control group was treated with conventional treatment+Low Molecular Weight Heparin Calcium,and the observation group was treated with conventional treatment+Low Molecular Weight Heparin Calcium+Gamma Globulin.The clinical therapeutic effect of two groups was compared.Result:The total effective rate of the observation group was significantly higher than that of the control group,the difference was statistically significant(P<0.05).The levels of CD4^(+),CD4^(+)/CD8^(+)and interferon-γ(IFN-γ)in the observation group after treatment were higher than those in the control group,while the levels of CD8^(+),interleukin-4(IL-4),fibrinogen(FIB),activated partial thromboplastin time(APTT),prothrombin time(PT),urinary microalbumin(mALB)andβ_(2)-microglobulin(β_(2)-MG)were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between two groups(P>0.05).Conclusion:The combination treatment of Low Molecular Weight Heparin Calcium and Gamma Globulin in children with Allergic purpura nephritis can effectively improve the immune function and coagulation function,reduce the inflammation,and has less adverse reactions,high safety,clinical effect is significant.
