益肾补骨汤联合经皮椎体成形术治疗骨质疏松性胸腰椎压缩骨折的临床效果

摘　　要：目的:探讨益肾补骨汤联合经皮椎体成形术(PVP)治疗骨质疏松性胸腰椎压缩骨折的临床效果。方法:选取2020年10月-2022年7月泉州市中医院收治的98例骨质疏松性胸腰椎压缩骨折患者,所有患者均进行PVP治疗,按照术后干预方法不同将患者分成观察组(n=49)和对照组(n=49),对照组术后给予常规抗骨质疏松治疗,观察组在对照组基础上加用益肾补骨汤治疗,比较两组临床治疗效果、骨密度、Cobb角、Oswestry功能障碍指数(ODI)、疼痛[数字评分法(NRS)]评分及并发症发生情况。结果:观察组临床治疗总有效率优于对照组,差异有统计学意义(P<0.05)。观察组治疗后3个月骨密度高于对照组,Cobb角小于对照组,ODI评分低于对照组,差异有统计学意义(P<0.05)。观察组治疗后1周、1个月及3个月NRS评分低于对照组,差异有统计学意义(P<0.05)。观察组治疗后并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:骨质疏松性胸腰椎压缩骨折患者采用益肾补骨汤联合PVP治疗可改善临床指征,降低功能性障碍,减轻术后疼痛,满足临床治疗目的。Objective:To investigate the clinical efficacy of Yishen Bugu Decoction combined with percutaneous vertebroplasty(PVP)in the treatment of osteoporotic thoracolumbar compression fracture.Method:A total of 98 patients with osteoporotic thoracolumbar compression fracture who admitted to Quanzhou Hospital of Traditional Chinese Medicine from October 2020 to July 2022 were selected,all patients were treated with PVP,they were divided into the observation group(n=49)and the control group(n=49)according to different postoperative intervention methods.The control group was given conventional anti-osteoporosis treatment after surgery,and the observation group was additionally treated with Yishen Bugu Decoction on the basis of the control group.The clinical treatment effect,bone mineral density,Cobb angle,Oswestry dysfunction index(ODI),pain[numerical rating scale(NRS)]score and complications were compared between two groups.Result:The clinical total effective rate of the observation group was better than that of the control group,the difference was statistically significant(P<0.05).The bone mineral density of the observation group at 3 months after treatment was higher than that of the control group,the Cobb angle was smaller than that of the control group,and the ODI score was lower than that of the control group,the differences were statistically significant(P<0.05).The NRS scores of the observation group at 1 week,1 month and 3 months after treatment were lower than those of the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Yishen Bugu Decoction combined with PVP in the treatment of patients with osteoporotic thoracolumbar compression fracture can improve the clinical indications,reduce functional disorders,relieve postoperative pain and meet the clinical therapeutic objectives.
