摘 要:目的 探讨3D打印技术辅助微导管塑形在颅内动脉瘤栓塞术中的应用价值。方法 纳入2020年5月至2022年1月自贡市第四人民医院采用3D打印辅助微导管塑形技术进行颅内动脉瘤栓塞治疗的32例患者(32枚动脉瘤),评判微导管塑形效果及动脉瘤栓塞结果。结果 32例患者微导管塑形均满意,一次到位成功,无重复塑形患者。26枚动脉瘤采用回撤技术使微导管落入动脉瘤腔,6枚动脉瘤通过微导丝引导入动脉瘤腔。所有患者栓塞术中微导管形态保持稳定,无微导管脱出事件。术后即刻血管造影显示,动脉瘤完全栓塞24枚,近全栓塞5枚,不全栓塞3枚。随访3个月,改良Rankin评分量表(mRS)评分0~2分29例,3~6分3例。结论 3D打印技术辅助微导管塑形方法应用于颅内动脉瘤栓塞术安全有效,有助于准确塑形微导管,顺利推进手术进程。Objective To explore the application value of three-dimensional(3-D)printing technologyassisted microcatheter shaping in interventional embolization treatment of intracranial aneurysm.Methods A total of 32 patients with intracranial aneurysm(32 aneurysms in total),who were admitted to the Zigong Municipal Fourth People's Hospital of China between May 2020 and January 2022 to receive interventional arterial embolization treatment with the help of 3-D printing technology-assisted microcatheter shaping,were enrolled in this study.The effect of microcatheter shaping and the results of aneurysm embolization were evaluated.Results In all the 32 patients,satisfactory microcatheter shaping was obtained and successful catheterization of the microcatheter into the targeted site with single procedure was accomplished.For 26 aneurysms,a microcatheter was inserted into the aneurysm cavity by using a pullback technique,and for the other 6 aneurysms,the microcatheter was inserted into the aneurysm cavity by using guide-wire guiding technique.In all patients,the morphology of the microcatheter remained stable during the embolization procedure,and no events of microcatheter dislocation occurred.Immediate postoperative angiography revealed that complete embolization was obtained in 24 aneurysms,near complete embolization in 5 aneurysms,and incomplete embolization in 3 aneurysms.The modified Rankin Scale(mRS)score of 0-2 points was obtained in 29 patients and 3-6 points in 3 patients.Conclusion In interventional embolization treatment for intracranial aneurysms,3-D printing technology-assisted microcatheter shaping is clinically safe and effective,it helps to accurately shape the microcatheter,then,the accurately shaped microcatheter can be smoothly inserted into the target site.
