内镜下喷洒安胃宁血散治疗非静脉曲张性上消化道出血的疗效观察

摘　　要：目的 探讨在内镜下喷洒安胃宁血散治疗非静脉曲张性上消化道出血的疗效观察。方法 将80例中医辨证属胃热炽盛证的非静脉曲张性上消化道出血患者,按照随机数字表法分为对照组40例,给予内镜下喷洒去甲肾上腺素溶液治疗,观察组40例给予内镜下喷洒安胃宁血散治疗,观察2组临床疗效、即时止血成功率、再出血率、临床指标以及不良反应发生率等。结果 观察组总有效率(97.56%)高于对照组(80.00%);治疗后观察组即时止血成功率(97.50%)优于对照组(77.50%),同时其再出血率(2.50%)低于对照组(25.00%),平均止血时间(1.78±0.53)d,短于对照组的(3.11±1.01)d,发生呕吐、黑便次数均少于对照组;观察组不良反应发生率(9.76%)低于对照组(27.5%),2组以上观察指标差异均有统计学意义(P<0.05)。结论 内镜下喷洒安胃宁血散治疗非静脉曲张性上消化道出血能有效提高治疗效果,缩短止血时间,降低不良反应发生率。Objective:To study the therapeutic effect of Anweiningxue Powder spraying under endoscope in the treatment of non-varicose upper gastrointestinal bleeding.Methods:A total of 80 patients with non-varicose upper digestive tract hemorrhage with syndrome differentiation of stomach heat were divided into a control group(40 cases)and an observation group(40 cases)according to random number table method.The control group was treated by spraying norepinephrine solution under endoscope and the treatment group was treated by spraying Anweiningxue Powder under endoscope.The clinical effect,success rate of immediate hemostasis,rebleeding rate,clinical indexes and incidence of adverse reactions were observed in 2 groups.Results:The total effective rate of the observation group(97.56%)was higher than that of control group(80.00%).After treatment,the success rate of immediate hemostasis in the observation group(97.50%)was better than that in the control group(77.50%),while the rebleeding rate in the treatment group(2.50%)was lower than that in the control group(25.00%).And the mean time of hemostasis was(1.78±0.53)d,which was shorter than that in the control group(3.11±1.01)d.The frequency of vomiting and black stool were less than that in the control group.The incidence of adverse reactions in the observation group(9.76%)was lower than that in the control group(27.5%),and there were statistically significant differences between the two groups(P<0.05).Conclusion:Anweiningxue Powder spraying under endoscope can effectively improve the therapeutic effect,shorten the hemostatic time and reduce the incidence of adverse reactions in the treatment of non-varicose upper gastrointestinal bleeding.
