杜仲颗粒联合拉贝洛尔对妊娠期高血压患者血压、妊娠结局及血浆PTX-3水平的影响

摘　　要：目的探讨杜仲颗粒联合拉贝洛尔对妊娠期高血压患者血压控制、妊娠结局及血浆PTX-3水平的影响。方法选取2020年5月至2022年5月收治的妊娠期高血压患者90例,随机分为对照组和联合组,每组45例。对照组给予拉贝洛尔治疗,联合组给予杜仲颗粒联合拉贝洛尔治疗。比较2组临床疗效,血流动力学指标[中心静脉压(CVP)、心脏指数、外周阻力],血压水平[收缩压(SBP)、舒张压(DBP)],不良妊娠结局,血浆五聚蛋白3(PTX3)、血清同型半胱氨酸(Hcy)、β-2微球蛋白(Β2-MG)水平,凝血酶指标[活化部分凝血酶原时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)]及不良反应情况。结果联合组总有效率为95.56%明显高于对照组77.78%(P<0.05);与治疗前比较,2组治疗后CVP、外周阻力、SBP、DBP、血浆PTX3、Hcy、Β2-MG、FIB水平均明显降低(P<0.05),与对照组比较,联合组明显降低(P<0.05);2组心脏指数、APTT、TT明显升高(P<0.05),与对照组比较,联合组明显升高(P<0.05);联合组新生儿窒息、产后出血、剖宫产等不良妊娠结局发生率为6.67%明显低于对照组的24.44%(P<0.05);2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论杜仲颗粒联合拉贝洛尔治疗妊娠期高血压患者可有效控制血压,改善妊娠结局及凝血酶指标,降低血浆PTX3、Hcy、Β2-MG水平,调节血流动力学指标,安全有效。Objective To investigate the influences of Duzhong Granules combined with labetalol on blood pressure control,pregnancy outcomes and plasma pentraxin 3(PTX 3)level in patients with gestational hypertension.Methods A total of 90 patients with gestational hypertension treated in our hospital from May 2020 to May 2022 were included.They were randomly grouped into the control group(45 cases)and the combined group(45 cases).Labetalol was given to all patients,and those in the combined group were additionally given Duzhong Granules.The clinical efficacy,hemodynamic indexes(central venous pressure[CVP],cardiac index,peripheral resistance),blood pressure levels(systolic blood pressure[SBP],diastolic blood pressure[DBP]),adverse pregnancy outcomes,plasma PTX3 level,serum homocysteine(Hcy),β-2 microglobulin(β2-MG),coagulation indexes(activated partial thromboplastin time[APTT],thrombin time[TT],fibrinogen[FIB])and adverse events were compared between groups.Results The total effective rate in the combined group was significantly higher than that in the control group(95.56%vs 77.78%,P<0.05).Compared with those before treatment,CVP,peripheral resistance,SBP,DBP,plasma PTX3,Hcy,Β2-MG,and FIB in both groups were significantly reduced after treatment(P<0.05),which were significantly lower in the combined group than those in control group(P<0.05).The cardiac index,APTT,and TT in both groups were significantly enhanced after treatment(P<0.05),which were significantly higher in the combined group than those in control group(P<0.05).The incidence of adverse pregnancy outcomes like neonatal asphyxia,postpartum hemorrhage,and cesarean section in the combined group was significantly lower than thosein control group(6.67%vs 24.44%,P<0.05).There was no significant difference in the incidence of adverse events between groups(P>0.05).Conclusion Duzhong Granules combined with labetalol can effectively control blood pressure,improve pregnancy outcomes and coagulation indexes,reduce plasma PTX3,HcyandΒ2-MG levels,and regulate hemodynamic
