摘　　要：目的探讨肺结核患者营养支持治疗的有效性及对患者营养状况、代谢反应的影响。方法选取2018年1月至2020年9月收治的肺结核患者100例,依据治疗方案不同分为对照组和研究组,每组50例。对照组给予常规治疗,研究组给予常规治疗+营养支持治疗。对比2组患者营养状态水平代谢指标前白蛋白、免疫能力以及不良反应情况。结果干预后2组患者白蛋白(ALB)、血红蛋白(Hb)、体重指数(BMI)、前白蛋白(PA)水平均有效改善,研究组上述指标水平高于对照组(P<0.05);干预前2组患者总胆固醇(TC)、空腹血糖(FBG)、内脏脂肪面积(VFA)水平差异无统计学意义(P>0.05),干预后2组上述指标水平均改善,且研究组患者TC、FBG、VFA水平均高于对照组(P<0.05);干预前2组T淋巴细胞因子水平差异无统计学意义(P>0.05),干预后研究组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平高于对照组,CD8^(+)水平低于对照组(P<0.05);研究组不良反应总发生率为12.0%低于对照组的30.0%(P<0.05)。结论在肺结核患者的临床治疗中,辅以营养支持治疗,可有效改善患者机体营养状况、提升代谢指标水平,还能增强患者机体免疫能力,减少不良反应,值得借鉴。Objective To explore the effectiveness of nutritional support treatment on pulmonary tuberculosis patients and its influence on the nutritional status and metabolic response.Methods A total of 100 pulmonary tuberculosis patients admitted to our hospital from January 2018 to September 2020 were selected for research.According to different treatment plans,patients were divided into control group and study group,with 50 cases per group.Patients in the control group were treated with conventional treatment,and those in the study group were treated with conventional treatment+nutritional support treatment.Nutritional status,metabolic indexes,immunity and adverse events were compared between groups.Results The albumin(ALB),hemoglobin(Hb),body mass index(BMI),and prealbumin(PA)levels of patients in both groups were significantly enhanced after treatment,which were significantly higher in the study group than in the control group(P<0.05).There were no significant differences in the levels of total cholesterol(TC),fasting blood glucose(FBG),and visceral fat area(VFA)before treatment between the two groups(P>0.05),which were all improved after treatment,and significantly higher in the study group than those in the control group(P<0.05).There was no significant difference in the levels of T-lymphocyte cytokines before treatment between the two groups(P>0.05).After treatment,the CD3+,CD4+,and CD4+/CD8+levels in the study group were significantly higher than those in the control group,and the CD8+level was significantly lower than that in the control group(P<0.05).The overall incidence of adverse events in the study group was significantly lower than that in the control group(P<0.05).Conclusion For pulmonary tuberculosis patients,nutritional support treatment can assist in improving the nutritional status,metabolic levels and immunity with a lowered incidence of adverse events.It is worth learning.
