摘　　要：目的探究微小核糖核酸(miR)-146a与c-Jun氨基末端激酶(JNK)在成人过敏性紫癜(HSP)患者外周血单个核细胞中的水平及临床意义。方法选取2020年1月至2022年12月开封市人民医院收治的50例成人HSP患者作为研究组(HSP组)。选取同期与HSP组性别、年龄组成相匹配,且于医院体检的健康成年人50例作为正常对照组(Normal组)。收集HSP组与Normal组人群外周血样本,检测两组外周血免疫球蛋白(Ig)A、IgG、IgM水平。分离外周血单个核细胞,通过实时荧光定量聚合酶链式反应检测HSP组与Normal组外周血单个核细胞中miR-146a、JNK表达情况,通过流式细胞术检测淋巴细胞亚群。通过相关性分析,探究miR-146a与JNK水平的相关性以及miR-146a、JNK与淋巴细胞亚群、IgA、IgG、IgM等免疫学指标的相关性。结果与Normal组相比,HSP组患者外周血单个核细胞中miR-146a水平降低,JNK水平升高(P<0.05),miR-146a水平与JNK水平呈负相关(P<0.05);HSP组IgA水平、CD8^(+)T细胞比率、Th2细胞比率、Th17细胞比率高于Normal组(P<0.05),CD4^(+)T细胞比率、Th1细胞比率、Treg细胞比率低于Normal组(P<0.05);HSP组患者外周血单个核细胞中miR-146a水平与IgA水平、CD8^(+)T细胞比率、Th2细胞比率、Th17细胞比率呈负相关(P<0.05),与CD4^(+)T细胞比率、Th1细胞比率、Treg细胞比率呈正相关(P<0.05)。JNK水平与CD4^(+)T细胞比率、Th1细胞比率、Treg细胞比率呈负相关(P<0.05),与IgA水平、CD8^(+)T细胞比率、Th2细胞比率、Th17细胞比率呈正相关(P<0.05)。结论miR-146a在成人HSP患者外周血单个核细胞中呈低表达,JNK呈高表达,二者呈负相关,两者可能通过相互作用,在HSP的发病中发挥重要作用。Objective To explore the level and correlation of microRNA-146a(miR-146a)and c-Jun N-terminal kinase(JNK)in peripheral blood mononuclear cell of adult patients with Henoch-Schoenlein purpura(HSP).Methods Selecting 50 adult patients with HSP admitted to Kaifeng People’s Hospital from January 2020 to December 2022 as the HSP group.Selecting 50 healthy adult subjects who underwent physical examination in hospital during the same period and match their gender and age composition with the HSP group as the Normal group.Collecting peripheral blood samples from both groups and detecting the levels of immunoglobulin(Ig)A,IgG,IgM in the peripheral blood of both groups.Isolating peripheral blood mononuclear cells and using real-time fluorescence quantitative PCR to detect the expression of miR-146a and JNK in peripheral blood mononuclear cells of the HSP group and Normal group,and using flow cytometry to detect lymphocyte subgroups.Through correlation analysis,to explore the correlation between miR-146a and JNK expression levels and lymphocyte subgroups,IgA,IgG,IgM and other immunological indicators.Results Compared with the Normal group,the level of miR-146a in peripheral blood mononuclear cells of the HSP group was decreased,while the level of JNK was increased(P<0.05),and the level of miR-146a was negatively correlated with JNK(P<0.05).The levels of IgA,CD8^(+)T cell proportion,Th2 cell proportion and Th17 cell proportion in the HSP group were higher than those in the Normal group(P<0.05),while the CD4^(+)T cell proportion,Th1 cell proportion and Treg cell proportion were lower than those in the Normal group(P<0.05).The expression level of miR-146a in peripheral blood mononuclear cells of HSP group was negatively correlated with IgA level,CD8^(+)T cell proportion,Th2 cell proportion and Th17 cell proportion(P<0.05),and positively correlated with CD4^(+)T cell proportion,Th1 cell proportion and Treg cell proportion(P<0.05).The expression level of JNK was negatively correlated with CD4^(+)T cell proportion,Th1 cell propo
