氟替美维吸入粉雾剂在COPD稳定期患者中的应用效果

摘　　要：目的探讨氟替美维吸入粉雾剂(FF)在慢性阻塞性肺疾病(COPD)稳定期患者中的作用效果。方法对2020年1月至2022年8月于南阳市第一人民医院接受治疗的90例COPD稳定期患者进行研究,按随机数表法分为对照组(n=45,接受呼吸内科常规治疗)和试验组(n=45,在常规治疗基础上接受FF治疗)。比较两组患者治疗效果,观察治疗前后患者血气指标、肺功能、呼吸困难程度、6 min步行试验(6MWD)、生活质量,记录不良反应发生率。结果试验组治疗总有效率高于对照组(P<0.05)。治疗后,两组患者动脉血氧分压(PaO_(2))、动脉血氧饱和度(SaO_(2))、第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)、肺总量(TLC)、6MWD水平提升,且试验组高于对照组(P<0.05),两组患者动脉血二氧化碳分压(PaCO_(2))水平、呼吸困难量表(mMRC)评分、成人慢阻肺生活质量量表(COPD-QOL)中各维度评分降低,且试验组低于对照组(P<0.05);两组患者未出现明显不良反应。结论对COPD稳定期患者给予FF治疗效果显著,有助于改善患者血气指标、肺功能及呼吸困难症状,提高运功能力和生活质量,适合临床推广应用。Objective To explore the effect of fluticasone furoate,umeclidinium bromide and vilanterol trifenatate powder for inhalation(FF)in the treatment of patients with stable chronic obstructive pulmonary disease(COPD).Methods A total of 90 patients with stable COPD who received treatment in Nanyang First People’s Hospital from January 2020 to August 2022 were studied.According to the random number table method,the patients were randomly divided into control group(n=45,received conventional respiratory medicine treatment)and experimental group(n=45,received FF treatment on the basis of conventional treatment).The therapeutic effect was compared between the two groups,and the blood gas indicators,pulmonary function,dyspnea degree,six-minute walk distance(6MWD)and quality of life were observed before and after treatment,and incidence rates of adverse reactions were recorded.Results The total effective rate of treatment in experimental group was higher than that in control group(P<0.05).After treatment,the arterial partial pressure of oxygen(PaO_(2)),arterial oxygen saturation(SaO_(2)),forced expiratory volume in one second(FEV_(1)),forced vital capacity(FVC),total lung capacity(TLC)and 6MWD were increased in both groups,and experimental group was higher than control group(P<0.05).The arterial partial pressure of carbon dioxide(PaCO_(2)),modified British Medical Research Council dyspnea scale(mMRC)score and adults COPD quality of life(COPD-QOL)score in the two groups were decreased,and experimental group was lower than control group(P<0.05).There were no obvious adverse reactions in the two groups.Conclusion FF has a significant therapeutic effect on patients with stable COPD,and it can help to improve the blood gas indicators,pulmonary function and dyspnea symptoms,and enhance the exercise ability and quality of life,thus it is suitable for clinical promotion and application.
