摘 要:目的探讨目标管理理论下的督导式预警性干预在慢性阻塞性肺疾病(COPD)合并呼吸衰竭通气治疗患者中的应用效果。方法采用前瞻性研究,选择2022年1—12月河南省人民医院收治的86例COPD合并呼吸衰竭患者,根据随机数字表法分为两组,各43例。所有患者均接受机械通气治疗,对照组接受常规督导式预警性护理干预,观察组在对照组基础上接受基于目标管理理论的督导式预警性干预。比较两组康复进程、血气指标及并发症。结果观察组机械通气时间、住ICU时间、住院时间均短于对照组(P<0.05);干预后,两组动脉血二氧化碳分压(PaCO_(2))较干预前降低,动脉血氧分压(PaO_(2))较干预前升高,且观察组PaCO_(2)水平更低,PaO_(2)水平更高(P<0.05)。观察组并发症总发生率低于对照组(P<0.05)。结论基于目标管理理论的督导式预警性干预可缩短COPD合并呼吸衰竭患者机械通气治疗时间,改善血气指标,同时可降低并发症发生率,促进患者康复。Objective To explore the effect of supervisory early warning intervention based on objectives management theory in ventilation treatment of patients with chronic obstructive pulmonary disease(COPD)and respiratory failure.Methods A prospective cohort study was conducted.A total of 86 patients with COPD complicated with respiratory failure who were admitted to Henan Provincial People’s Hospital from January to December 2022 were selected.They were divided into two groups according to the random number table method,with 43 patients in each group.All patients received mechanical ventilation treatment.The control group received routine supervision early warning nursing intervention,and the observation group received supervision early warning intervention based on objectives management theory on the basis of the control group.The rehabilitation progress,blood gas indicators and complications were compared between the two groups.Results The observation group had shorter mechanical ventilation time,ICU stay time and hospitalization time than the control group(P<0.05).After the intervention,the arterial blood partial pressure of carbon dioxide(PaCO_(2))in the two groups decreased compared to before the intervention,while the arterial blood partial pressure of oxygen(PaO_(2))increased compared to before the intervention,and the PaCO_(2) in the observation group was lower,and the PaO_(2) was higher(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion The supervisory early warning intervention based on objectives management theory can shorten the time of mechanical ventilation treatment for COPD patients with respiratory failure,improve blood gas indicators,reduce the incidence of complications,and promote the recovery of patients.
