天麻钩藤饮合葛根芩连汤加减方联合氢溴酸加兰他敏改善脑小血管病认知功能障碍的疗效

摘　　要：目的 观察天麻钩藤饮合葛根芩连汤加减方联合氢溴酸加兰他敏改善脑小血管病(cerebral small vessel disease,CSVD)认知功能障碍的效果。方法 选取2021年12月—2022年6月北京市中医医院怀柔医院神经内科收治的90例CSVD认知功能障碍患者,按照就诊顺序分为观察组与对照组,每组45例。最终观察组完成40例,对照组完成42例。对照组患者给予氢溴酸加兰他敏治疗,观察组患者在对照组治疗的基础上联合天麻钩藤饮合葛根芩连汤加减方治疗。比较两组治疗效果。结果 治疗后,两组智能精神状态检查量表、蒙特利尔认知评估量表评分及日常生活活动能力评分均高于治疗前,且观察组各量表评分均高于对照组,差异有统计学意义(P<0.05)。治疗前后,两组患者丙氨酸氨基转移酶、天门冬氨酸氨基转移酶、血肌酐、尿素氮及肾小球滤过率比较,差异无统计学意义(P>0.05)。两组患者不良反应总发生率比较,差异无统计学意义(P>0.05)。结论 天麻钩藤饮合葛根芩连汤加减方联合氢溴酸加兰他敏可明显改善CSVD认知功能,提高日常生活能力,且不良反应少,值得临床研究。Objective To observe the effect of Tianma Gouteng decoction combined with Gegen Qinlian decoction combined with combined with galantamine hydrobromide on cognitive dysfunction of cerebral small vessel disease(CSVD).Methods 90 CSVD cognitive impairment patients admitted to the Neurology Department of Huairou Hospital of Beijing Traditional Chinese Medicine Hospital from December 2021 to June 2022 were selected and divided into an observation group and a control group according to the order of treatment,with 45 patients in each group.The final observation group completed 40 cases,while the control group completed 42 cases.The control group patients were treated with galantamine hydrobromide,while the observation group patients were treated with a combination of Tianma Gouteng Yin and Gegen Qinlian Tang modified formula on the basis of the control group treatment.Compared the treatment effects between the two groups.Results After treatment,the scores of MMSE,MoCA scales,and Barthel index in both groups were higher than before treatment,and the scores of each scale in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).Before and after treatment,there was no statistically significant difference in ALT,AST,Scr,BUN,and GFR between the two groups of patients(P>0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups of patients(P>0.05).Conclusion Tianma Gouteng decoction combined with Gegen Qinlian Decoction combined with galantamine hydrobromide can significantly improve the cognitive function of CSVD,improve the ability of daily living,and less adverse reactions,which is worthy of further clinical research and promotion.
