摘 要:目的 分析膝关节交叉韧带损伤诊治重建中磁共振成像磁共振成像(magnetic resonance imaging,MR)及电子计算机断层扫描(computed tomography,CT)的应用价值。方法 选取2019年1月—2022年12月江苏省句容市人民医院收治的80例疑似膝关节交叉韧带损伤患者,依次进行MR、CT检查,以《前交叉韧带损伤临床诊疗循证指南(2022版)》诊断结果为金标准,对比MR、CT诊断效能。测量并对比前交叉韧带损伤、后交叉韧带损伤相关指标。结果 CT诊断准确度为87.50%,低于MR诊断准确度96.25%,差异有统计学意义(χ^(2)=4.102,P<0.05);CT诊断敏感度、特异度分别为91.18%、66.67%,MR分别为97.06%、91.67%,差异无统计学意义(χ^(2)=1.195、1.010,P>0.05)。MR与CT检查前交叉韧带损伤的前内侧束长度/夹角、后外侧束长度/夹角、厚度、宽度对比,差异无统计学意义(P>0.05)。MR与CT检查后交叉韧带损伤的前外侧束长度/夹角、后内侧束长度/夹角、厚度、宽度对比,差异无统计学意义(P>0.05)。结论 与CT相比,MR对膝关节交叉韧带损伤患者诊断优势更明显。Objective To analyze the application value of magnetic resonance imaging(MR)and computed tomography(CT)in the diagnosis and treatment of cruciate ligament injury of knee joint.Methods A total of 80 patients with suspected cruciate ligament injury in Jurong People's Hospital of Jiangsu Province from January 2019 to December 2022 were enrolled.MR and CT examinations were performed successively,and the diagnostic results of Evidence-based Guidelines for Clinical Diagnosis and Treatment of Anterior cruciate Ligament Injury(2022 edition)were taken as the gold standard.The diagnostic efficiency of MR and CT was compared.The index of anterior cruciate ligament injury and posterior cruciate ligament injury were measured and compared.Results The diagnostic accuracy of CT was 87.50%,lower than 96.25%of MR,the difference was statistically significant(χ^(2)=4.102,P<0.05).There was no statistically significant differencehe in diagnostic sensitivity and specificity of CT(91.18%and 66.67%)and MR(97.06%and 91.67%)(χ^(2)=1.195,1.010,P>0.05).There was no statistically significant difference between MR and CT in anterior medial tract length/angle,posterior lateral tract length/angle,thickness and width(P>0.05).There was no statistically significant difference between MR and CT in the length/angle of anterior lateral fasciculus,length/angle of posterior me⁃dial fasciculus,thickness and width of posterior cruciate ligament injury(P>0.05).Conclusion Compared with CT,MR has more advantages in the diagnosis of cruciate ligament injury of knee joint.
