血清指标联合应用对糖尿病诊断价值的分析

摘　　要：目的 分析血清谷氨酸脱羧酶(glutamic acid decarboxylase,GAD)、胰岛素自身抗体(insulin autoantibody,IAA)、胰岛细胞抗体(islet cell antibody,ICA)、C肽(C-peptide,C-P)联合应用对糖尿病的诊断价值。方法 选取2022年2月—2023年2月麻城市人民医院收治的80例糖尿病患者为糖尿病组,另选取同期80例健康体检者为对照组,检测两组血清GAD、IAA、ICA和C-P水平,经受试者工作特征曲线(receiver operating characteristic curves,ROC)分析联合检测多项指标对糖尿病的诊断价值。结果 糖尿病组的GAD、IAA、ICA和C-P水平明显高于对照组,差异有统计学意义(P<0.05)。根据糖尿病患者GAD、IAA、ICA和C-P检测数据,制作ROC曲线,结果显示,血清指标联合应用诊断糖尿病的灵敏度为93.75%,特异度为86.25%,高于单一检测。结论糖尿病患者GAD、IAA、ICA和C-P水平处于异常状态,血清GAD、IAA、ICA和C-P联合诊断糖尿病的诊断效能高于单一检测。Objective To analyze the diagnostic value of serum glutamic acid decarboxylase(GAD),insulin autoantibody(IAA),islet cell antibody(ICA) and C-peptide(C-P) in diabetes.Methods 80 patients with diabetes admitted to Macheng People's Hospital from February 2022 to February 2023 were selected as the diabetes group,and 80 healthy people in the same period were selected as the control group.Serum GAD,IAA,ICA and C-P levels of the two groups were detected.Receiver operating characteristic curves(ROC) were used to analyze the diagnostic value of multiple indicators of joint detection for diabetes.Results The levels of GAD,IAA,ICA and C-P in the diabetes group were significantly higher than those in the control group,the difference was statistically significant(P<0.05).The ROC curve was made according to the test data of GAD,IAA,ICA and C-P in diabetes patients.The results showed that the sensitivity and specificity of the combination of serum indicators in the diagnosis of diabetes were 93.75% and 86.25% respectively,which were higher than those of a single test.Conclusion The levels of GAD,IAA,ICA and C-P in diabetes patients were abnormal,and the diagnostic efficacy of serum GAD,IAA,ICA and C-P in the diagnosis of diabetes was higher than that of single detection.
