摘  要:目的 分析经皮椎体成形骨水泥呈弥散型分布对患者骨密度、伤椎椎体高度的影响。方法 回顾性分析2020年1月—2023年2月盐城市第三人民医院骨科收治的64例老年骨质疏松性胸腰椎骨折患者的病历资料,所有患者均进行经皮椎体成形术,依据X射线片上骨水泥分布形态分为弥散不良组(n=28)、弥散分布组(n=36),比较两组手术情况,不同时间治疗指标,并发症发生率、伤椎邻近椎体再发骨折率。结果 两组骨水泥灌注量、骨水泥推注时间、手术时间、术中出血量、术后住院时间对比,差异无统计学意义(P>0.05)。术后3 d、1个月、3个月,两组治疗指标较术前均改善,差异有统计学意义(P<0.05);术后1、3个月,弥散分布组伤椎前缘高度、局部后凸Cobb角较弥散不良组更优,差异有统计学意义(P<0.05)。两组并发症发生率对比,差异无统计学意义(P>0.05);持续对患者随访3个月,弥散分布组伤椎邻近椎体再发骨折率(0)较弥散不良组(17.86%)更低,差异有统计学意义(χ^(2)=4.714,P<0.05)。结论 经皮椎体成形骨水泥弥散分布情况会对患者伤椎前缘椎体高度、局部后凸Cobb角造成一定影响,若弥散分布良好,还可降低伤椎邻近椎体再发骨折率。Objective To analyze the effect of diffuse distribution of percutaneous vertebroplasty bone cement on bone mineral density and height of injured vertebral body in patients.Methods The medical records of 64 elderly patients with osteoporotic thoracolumbra fracture admitted to the Department of Orthopaedics of Yancheng Third People's Hospital from January 2020 to February 2023 were retrospectively analyzed.All patients underwent percutaneous vertebrae plasty and were divided into poor diffusion group(n=28)and diffusion distribution group(n=36)according to the bone cement distribution morphology on X-ray films.The operation conditions,treatment indexes at different time,complication rate and recurrence rate of vertebral body near injured vertebra were compared between the two groups.Results There was no statistically significant difference in bone cement infusion volume,bone cement injection time,operation time,intraoperative blood loss and postoperative hospital stay between the two groups(P>0.05).At 3 days,1 month and 3 months after operation,the therapeutic indexes in both groups were improved compared with those before operation,and the difference was statistically significant(P<0.05).At 1 month and 3 months after surgery,the anterior edge height and local kyphotic Cobb Angle of the injured vertebrae in the diffuse distribution group were better than those in the poor diffusion group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).After 3 months of follow-up,the recurrence fracture rate of the injured vertebrae adjacent to the injured vertebrae in the diffuse distribution group(0)was lower than that in the poor diffusion group(17.86%),and the difference was statistically significant(χ^(2)=4.714,P<0.05).Conclusion The diffusion distribution of percutaneous vertebroplasty bone cement will have a certain impact on the height of the vertebral body at the front edge of the injured vertebra and
