颅脑外伤患者行开颅血肿清除联合去骨瓣减压术对恢复期昏迷程度及并发症的影响

摘　　要：目的 探讨开颅血肿清除联合去骨瓣减压术治疗颅脑外伤患者对恢复期昏迷程度及并发症的影响。方法 方便选取2020年9月—2022年9月盐城市第三人民医院收治的112例颅脑外伤患者作为研究对象,按照数字表法随机分为两组,对照组56例给予单纯开颅血肿清除手术治疗,观察组56例给予开颅血肿清除+去骨瓣减压手术治疗,对患者术后随访3个月,观察记录两组患者并发症及恢复期昏迷程度。结果 观察组并发症发生率为10.71%低于对照组的25.00%,差异有统计学意义(χ^(2)=3.896,P<0.05)。观察组治疗总有效率为87.50%高于对照组的71.43%,差异有统计学意义(χ^(2)=4.432,P<0.05)。结论 开颅血肿清除联合去骨瓣减压术在颅脑外伤患者的治疗中效果显著,可有效改善患者恢复期昏迷程度,降低患者术后相关并发症的发生率,具有重要临床价值。Objective To explore the effect of craniotomy hematoma evacuation combined with decompressive craniectomy on the degree of coma and complications in the recovery period of patients with craniocerebral trauma.Methods 112 patients with traumatic brain injury admitted to the Third People's Hospital of Yancheng City from September 2020 to September 2022 were conveniently selected as the research subjects.They were randomly divided into two groups according to the number table method.The control group(56 cases) received simple craniotomy hematoma removal surgery,while the observation group(56 cases) received craniotomy hematoma removal and bone flap decompression surgery.The patients were followed up for 3 months after surgery,and the complications and degree of coma during recovery were observed and recorded in both groups.Results The incidence of complications in the observation group was 10.71% lower than that in the control group(25.00%),the difference was statistically significant(χ^(2)=3.896,P<0.05).The total effective rate of the observation group was 87.50%,which was higher than 71.43% of the control group,the difference was statistically significant(χ^(2)=4.432,P<0.05).Conclusion Craniotomy hematoma evacuation combined with decompressive craniectomy has a significant effect in the treatment of patients with craniocerebral trauma.It can effectively improve the degree of coma in the recovery period of patients,reduce the incidence of postoperative complications,and has important clinical value.
