摘 要:目的 探讨对孕足月产妇采用低位水囊引产联合静滴缩宫素方案的效果及对血清白细胞计数(white blood cell count,WBC)、C反应蛋白(C-reactive protein,CRP)的影响。方法 便利选取2020年2月—2023年2月江苏大学附属武进医院(徐州医科大学武进临床学院)收治的孕足月产妇96例,随机分为静滴缩宫素治疗的对照组(n=48)与联合低位水囊引产干预的观察组(n=48),比较两组分娩情况以及宫颈成熟情况,统计两组引产至临产时间、产程情况与产后2 h出血量评估分娩情况,对比两组分娩前后WBC、CRP水平评估炎症反应,并统计并发症发生率。结果 观察组阴道分娩率(91.67%)、促宫颈成熟有效率(97.92%),高于对照组的72.92%、81.25%,差异有统计学意义(χ^(2)=5.790、7.144,P<0.05)。观察组引产至临产时间、第一产程、第二产程、总产程、产后2 h出血量均优于对照组,差异有统计学意义(P<0.05)。观察组分娩后WBC、CRP均低于对照组,差异有统计学意义(t=4.528、5.317,P<0.05)。观察组并发症发生率明显低于对照组,差异有统计学意义(χ^(2)=4.019,P<0.05)。结论 对孕足月产妇采用低位水囊引产联合静滴缩宫素方案能提高阴道分娩率,可促进宫颈成熟,能缩短分娩时间,减少出血量,可减轻炎症反应,降低并发症风险。Objective To investigate the effect of low water sac induced labor combined with intravenous drip of oxytocin in full-term pregnant women on serum white blood cell count(WBC) and C-reactive protein(CRP).Methods 96 full-term pregnant women admitted to Wujin Hospital Affiliated to Jiangsu University(Wujin Clinical College of Xuzhou Medical University) from February 2020 to February 2023 were conveniently selected and randomly divided into intravenous drip of oxytocin control group(n=48) and combined with low water sac induced labor intervention observation group(n=48).Labor and cervical maturity of the two groups were compared,and the time from labor induction to labor,labor process and 2-hour postpartum blood loss of the two groups were evaluated.The levels of WBC and CRP before and after delivery were compared between the two groups to evaluate the inflammatory response,and the incidence of complications was calculated.Results The vaginal delivery rate(91.67%) and effective rate of promoting cervical maturation(97.92%) of the observation group,which were higher than those of the control group(72.92%) and(81.25%),and the difference was statistically(χ^(2)=5.790,7.144,P<0.05).The observation group showed better induction to labor time,first stage,second stage,total stage,and 2-hour postpartum bleeding compared to the control group,and the difference was statistically(P<0.05).After delivery,the WBC and CRP in the observation group were lower than those in the control group,and the difference was statistically(t=4.528,5.317,P<0.05).The incidence of complications in the observation group was significantly lower than that in the control group,and the difference was statistically(χ^(2)=4.019,P<0.05).Conclusion For full-term pregnant women,low water sac induction of labor combined with intravenous drip of oxytocin can improve the rate of vaginal delivery,promote cervical maturity,shorten labor time,reduce blood loss,reduce inflammation and reduce the risk of complications.
