摘  要:目的 研究新生儿缺氧缺血性脑病严重程度与血乳酸、外周血循环内皮细胞的相关性。方法 选择本院2018年6月到2020年2月106例缺氧缺血性脑病患儿作为研究对象,其中轻度患儿35例,中度患儿36例,重度患儿35例。以本院同期足月健康新生儿35例为对照组,分组后,分析各组间胎龄、体质量等临床资料和外周血循环内皮细胞水平和血乳酸水平,有序多分类逻辑回归分析新生儿缺氧缺血性脑病严重程度与血乳酸、外周血循环内皮细胞的相关性。结果 新生儿缺氧缺血性脑病轻度组、中度组、重度组和健康对照组性别、胎龄、体质量等一般资料比较无统计学意义(P>0.05);四组患儿之间的外周血循环内皮细胞水平、血乳酸水平比较存在差异性(P<0.05),疾病不同严重程度外周血循环内皮细胞、血乳酸水平有差异(P<0.05);采用有序多分类logistic回归分析后,外周血循环内皮细胞、血乳酸水平与缺氧缺血性脑病严重程度呈正相关(P<0.05);ROC曲线分析显示,外周血循环内皮细胞、血乳酸水平预测新生儿缺氧缺血性脑病严重程度的曲线下面积(AUC)分别为0.975、0.992,敏感性分别为0.886、0.971,特异性分别为0.981、0.991,最佳截断点分别为1.400、5.585。结论 血乳酸、外周血循环内皮细胞水平对新生儿缺氧缺血性脑病具有重要影响,可通过监测患儿机体内两种指标水平的变化情况判断疾病发展状态,能够有效提高临床诊断、治疗效率。Objective To study the correlation between the severity of neonatal hypoxic-ischemic encephalopathy and blood lactic acid and peripheral blood circulating endothelial cells.Methods A total of 106 children with hypoxic-ischemic encephalopathy(HIE)from June 2018 to February 2020 were selected as the study subjects,including 35 children with mild,36 children with moderate and 35 children with severe.In the same period in our hospital full-term healthy newborns 35 cases as control group,group,each group gestational age,body mass and other clinical data and peripheral blood circulation endothelial cells and blood lactic acid level,more orderly severity classification of logistic regression analysis of neonatal hypoxic ischemic encephalopathy and blood lactic acid,the correlation of peripheral blood circulating endothelial cells.Results There were no significant differences in gender,gestational age and body weight between mild,moderate and severe neonatal hypoxic-ischemic encephalopathy group and healthy control group(P>0.05).There were differences in the levels of peripheral blood circulating endothelial cells and blood lactic acid among the four groups(P<0.05),and there were differences in the levels of peripheral blood circulating endothelial cells and blood lactic acid among the different severity of disease(P<0.05).The levels of circulating endothelial cells and lactic acid in peripheral blood were positively correlated with the severity of HYPOxic-ischemic encephalopathy(P<0.05).ROC curve analysis showed that the area under curve(AUC)of peripheral blood circulating endothelial cells and blood lactic acid levels in predicting the severity of neonatal hypoxic-ischemic encephalopathy was 0.975 and 0.992,the sensitivity was 0.886 and 0.971,and the specificity was 0.981 and 0.991,respectively.The best cut-off points were 1.400 and 5.585,respectively.Conclusion The levels of blood lactic acid and peripheral blood circulating endothelial cells have important effects on neonatal hypoxic ischemic encephalopathy.The devel
