摘  要:目的 观察快律宁(KLN)对豚鼠心室肌细胞延迟整流钾电流(IK)及内向整流钾电流(IK1)的影响,探讨KLN抗心律失常的作用机制。方法 Langendorff离体心脏灌流系统灌流消化分离出单个豚鼠心室肌细胞,采用全细胞膜片钳记录方法,依次给予低剂量KLN(2.5 mg/ml)、中剂量KLN(5 mg/ml)、高剂量KLN(10 mg/ml),于细胞外灌流药物5 min后,依次记录各剂量给药后电流值。以正常细胞外液冲洗细胞洗脱药物5 min,记录冲洗后电流值。观察KLN对IK及IK1的影响。结果 KLN可降低IK及其尾电流(IK,tail)的电流幅度,使电流密度-电压关系曲线下移;KLN能显著抑制IKs及IKs, tail,作用呈剂量依赖性。低剂量KLN对IK1无明显作用,中、高剂量KLN可显著降低IK1内向电流;高剂量KLN对IK1外向电流有抑制作用。结论 KLN对IK具有剂量依赖性抑制作用,高剂量KLN可抑制IK1内、外向电流,这可能是其发挥抗心律失常作用的机制之一。Objective To observe the effect of KLN on guinea pig ventricular myocytes delayed rectifier potassium current(I K)and inward rectifier potassium current(I K1),in order to explore the possible ionic mechanisms of KLN.Methods Single guinea pig ventricular myocytes were isolated by perfusion digestion in Langendorff isolated car-diac perfusion system.Whole cell patch clamp recording method was used.Low dose KLN(2.5mg/ml),medium dose KLN(5mg/ml)and high dose KLN(10mg/ml)were ad ministered successively,and the current value of each dose was recorded after 5min of extracellular perfusion.The cells were rinsed with normal extracellular solu-tion for 5min to eluate the drug,and the current value after rinsing was recorded.The effects of KLN on I K and I K1 were observed and recorded.Results KLN reduced I K and I K tail current(I K,tail),made the current density-volt-age relationship curve down.KLN significantly inhibited I K s and I K s,tial in a dose-dependent manner.Low dose KLN showed no significant effect on I K1.Medium and high dose KLN significantly reduced I K1 inward current.High doses KLN reduced I K1 outward current.Conclusion KLN has a dose-dependent inhibition on I K.High dose KLN also reduces the inward and outward current of I K1.Which may be the mechanisms of KLN′s antiarrhythmic effect.
