摘　　要：目的 根据文献检索分析成人大骨节病患者血清白细胞介素6(IL-6)、肿瘤坏死因子-α(TNF-α)含量的变化,探讨IL-6与TNF-α在大骨节病发病机制中的作用。方法 对中国知网、万方、维普、PubMed、Embase等数据库进行系统检索,起止时间为建库至2023年5月公开发表的关于成人大骨节病患者血清IL-6与TNF-α水平检测分析的文献,运用Revman 5.3软件进行统计学分析。结果 共纳入符合要求的研究文献16篇,成人大骨节病例数为782例,正常对照组610例。Meta分析结果显示大骨节病患者血清中IL-6与TNF-α水平均高于正常对照组,分别为SMD=2.02,95%CI:0.12~3.92,Z=2.09,P=0.04;SMD=2.75,95%CI:1.92~3.57,Z=6.53,P<0.000 01。结论 通过Meta分析,成人大骨节病患者血清IL-6与TNF-α水平明显升高,IL-6与TNF-α在成人大骨节病发病机制中具有一定作用,为今后大骨节病防治提供理论依据。Objective To analyze the changes of serum interleukin-6(IL-6) and tumor necrosis factor-α(TNF-α) in adult patients with Kashin-Beck disease based on literature review,and to investigate the role of IL-6 and TNF-α in the pathogenesis of Kashin-Beck disease.Methods A systematic search was conducted on Chinese National Knowledge Net,Wanfang,VIP,PubMed and Embase database,and all published literatures on detection of IL-6 and TNF-α in adult patients with Kashin-Beck disease from the establishment of the database to May 2023.Statistical analysis was performed by using Revman 5.3 software.Results A total of 16 literatures were included,including 782 adult Kashin-Beck patients and 610 healthy persons in normal control group.The results of Meta-analysis showed that levels of serum IL-6 and TNF-α levels in serum for patients with Kashin-Beck disease were higher than those in normal control group,namely SMD=2.02,95%CI 0.12-3.92,Z=2.09,P=0.04;SMD=2.75,95%CI 1.92-3.57,Z=6.53,P<0.000 01.Conclusion By Meta analysis,levels of IL-6 and TNF-α in serum are significantly increased in adult patients with Kashin-Beck disease,which are significant in the pathogenesis of Kashin-Beck disease.This will provide theoretical basis for Kashin-Beck disease prevention and treatment in the future.
