动静结合针法对中风后偏瘫患者运动功能及步态的影响

摘　　要：目的 观察动静结合针法对中风后痉挛性偏瘫患者的步态改善情况。方法 将纳入研究的60例中风后痉挛性偏瘫患者按照随机数字表法分为试验组和对照组,试验组使用动静结合针法治疗,对照组采用普通电针治疗,观察治疗前后2组的三维步态分析时空参数、改良Ashworth量表分级、tinetti步态量表分值。结果 2组患者治疗后的三维步态分析时空参数(步行周期、步幅、步宽、步频、摆动时相及总支撑时相)、tinetti步态量表分值较治疗前均有显著差异(P <0.05),且试验组治疗后三维步态分析时空参数改善情况显著优于对照组。结论 动静结合针法及普通电针均对中风后痉挛性偏瘫患者的步态有显著的改善效果,且动静结合针法效果明显优于普通电针。Objective To observe the improvement of gait in patients with spastic hemiplegia after stroke by combination of Dong and Jing acupuncture.Methods 60 patients with spastic hemiplegia after stroke who were included in the study were divided into the experiment group and the control group according to the random number table method.The experiment group was treated with combination of Dong and Jing acupuncture,and the control group was treated with ordinary electroacupuncture.The spatiotemporal parameters of three-dimensional gait analysis,Modified Ashworth Scale grading and Tinetti Performance-Oriented Mobility Assessment scale scores of the two groups before and after treatment were observed.Results The spatiotemporal parameters of three-dimensional gait analysis(walking cycle,stride length,stride width,stride frequency,swing phase,and total support phase)and Tinetti Performance-Oriented Mobility Assessment scale scores after treatment showed significant differences between the two groups compared with before treatment(P<0.05),and the improvement of the spatiotemporal parameters of three-dimensional gait analysis in the experimental group after treatment was significantly better than that in the control group.Conclusion Combination of Dong and Jing acupuncture and ordinary electroacupuncture have the significant improvement effects on gait in patients with spastic hemiplegia after stroke,and the effect of combination of Dong and Jing acupuncture is significantly better than that of ordinary electroacupuncture.
