聚氨酯泡沫敷料联合磺胺嘧啶银霜对深Ⅱ度烧伤患者创面溶痂的影响

摘　　要：目的探究聚氨酯泡沫敷料联合磺胺嘧啶银霜对深Ⅱ度烧伤患者创面溶痂的影响。方法选取2019年5月-2022年5月于聊城市第二人民医院收治的深Ⅱ度烧伤患者80例,随机分为对照组(38例)和研究组(42例)。对照组使用磺胺嘧啶银霜进行治疗,研究组采取在对照组的基础上联合聚氨酯泡沫敷料予以治疗。检测2组完全溶痂的时间以及溶痂率、完全治愈的时间以及治愈率、炎症指标、不同时间段的视觉模拟评分法(VAS)评分、并发症发生情况的变化。结果与对照组相比,研究组完全溶痂的时间、完全治愈的时间、并发症总的发生率下降,T1、T2、T3的溶痂率以及T1、T2、T3的治愈率上升,且研究组较对照组情况改善更明显,差异具有统计学意义(P<0.05);治疗7 d后,2组的白介素-1(IL^(-1))、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、不同时间段VAS评分有所下降,且研究组低于对照组,差异具有统计学意义(P<0.05)。结论通过聚氨酯泡沫敷料联合磺胺嘧啶银霜对于治疗深Ⅱ度烧伤患者效果明显,可有效提高患者的溶痂及治愈率,加快创面恢复,降低患者创面疼痛的程度。Objective To explore the effect of polyurethane foam dressing combined with sulfonazine silver cream on eschar dissolution in patients with deep second degree burns.Methods 80 patients with deep second degree burn admitted to the Second People’s Hospital of Liaocheng from May 2019 to May 2022 were selected and randomly divided into the control group(n=38)and the study group(n=42).The control group was treated with sulfonazine silver cream,and the study group was treated with polyurethane foam dressing on the basis of the treatment of the control group.The changes in the time of complete eschar dissolution,eschar dissolution rate,time to reach full cure,cure rate,inflammation index,visual analogue scale(VAS)scores at different time periods,and incidence of complications in two groups were detected.Results Compared with the control group,in the study group the time of complete eschar dissolution and time to reach full cure were shortened,and the total incidence of complications decreased,while the eschar dissolution rate in T1,T2,and T3,as well as the cure rate in T1,T2,and T3 increased,and the study group showed more significant improvement compared with the control group,with a statistically significant difference(P<0.05).After 7 days of treatment,interleukin-1(IL-1),interleukin-6(IL-6)and tumour necrosis factor-α(TNF-α)levels,and VAS scores at different time periods decreased,and the study group was lower than the control group,and there was statistically significant difference(P<0.05).Conclusion The polyurethane foam dressing combined with sulfonazine silver cream has obvious effect on the treatment of deep second degree burn patients,which can effectively improve the eschar dissolution and cure rate of patients,accelerate the recovery of wound,and reduce the degree of wound pain of patients.
