摘　　要：目的:观察人文关怀理念下全程护理对翼状胬肉切除+角膜缘干细胞移植术患者心理应激和术后恢复的影响。方法:选取2021年1月至2021年12月期间济南市第二人民医院拟行翼状胬肉切除+角膜缘干细胞移植术的110例患者,按随机双盲法分组为观察组55例和对照组55例,观察组患者接受人文关怀理念下全程护理,对照组患者接受常规围手术期护理。手术前后使用一般自我效能量表(GSES)、心理弹性量表(CD-RISC)、抑郁-焦虑-应激自评量表简版(DASS-21)、感知应激量表(PSS)、医学应对方式问卷(MCMQ)分别评估患者并记录两组患者术后创面愈合时间及术后并发症发生情况。结果:术后观察组患者GSES、CD-RISC及MCMQ的面对维度得分高于对照组,DASS-21各维度、PSS、MCMQ的回避和屈服维度得分、术后并发症总发生率均低于对照组,创面愈合时间短于对照组,差异有统计学意义(P<0.05)。结论:采用人文关怀理念下全程护理有助于减轻翼状胬肉切除+角膜缘干细胞移植术患者的心理应激,有利于促进其术后恢复。Objectivee To observe the effect of total nursing care under the concept of humanistic care on psychological stress and postoperative recovery in patients undergoing pterygium excision+corneal limbal stem cell transplantation.Methods A total of 110 patients in this Hospital from January 2021 to December 2021 who were undergone pterygium excision+corneal limbal stem cell transplantation as objects and divided into 55 cases observation group and 55 cases control group according to the randomized double-blind method,and the patients in the observation group received the whole process of nursing care under the concept of humanistic care,while the patients in the control group received routine perioperative nursing care.Before and after surgery,the General Self-Efficacy Scale(CSES),the Psychological Resilience Scale(CD-RISC),the Depression-Anxiety-Stress Self-Rating Scale Short Version(DASS-21),the Perceived Stress Scale(PSS),and the Medical Coping Mode Questionnaire(MCMQ)were used to assess the patients and record the postoperative wound healing time and the occurrence of postoperative complications in both groups,respectively.Results Postoperatively,the patients in the observation group had higher scores on the GSES,CD-RISC and MCMQ's confrontation dimension than those in the control group,and the scores on the DASS-21 dimensions,PSS,MCMQ's avoidance and yielding dimensions,and the total incidence of postoperative complications were lower than those in the control group,and the trauma healing time was shorter than that in the control group,and the difference was statistically significant(P<0.05).Conclusions Adopting the concept of humanistic care in the whole process of nursing helps to reduce the psychological stress of pterygium excision+corneal limbal stem cell transplantation patients,which is conducive to the promotion of their postoperative recovery.
