摘　　要：目的了解慢性阻塞性肺疾病(COPD)不同程度分级对老年人生活质量的影响。方法根据慢性阻塞性肺疾病全球倡议的标准(GOLD),将符合纳入标准的567例年龄≥60岁COPD患者分为4级,收集老人的基本资料,并进行健康状况量表(SF-36)的问卷调查。结果本研究纳入的567例COPD患者中,男性395例(67.9%),女性172例(30.3%);Ⅰ级患者有319人(56.3%),Ⅱ级患者有204人(36.0%),Ⅲ级患者有38人(6.7%),Ⅳ级患者有6人(1.1%)。单因素线性回归分析显示,年龄、性别与COPD分级是老人生活质量的影响因素,差异有统计学意义(P<0.05);不同分级组间分析显示,Ⅰ级与Ⅱ级的患者生活质量无明显差异(P>0.05),Ⅰ级、Ⅱ级、Ⅲ级、Ⅳ级患者的生活质量比较差异具有统计学意义(P<0.05)。结论年龄、性别及COPD分级是老年COPD患者的生活质量的影响因素,且与分级程度呈负相关关系。Objective To investigate the effects of different grades of chronic obstructive pulmonary disease(COPD)on the quality of life in the elderly.Methods According to the global initiative for GOLD,567 patients with COPD older than or equal to 60 years old who met the inclusion criteria were divided into four levels.Base data of the elderly were collected,and the SF-36 questionnaire survey was conducted.Results There were total 567 individuals,male 395(69.7%),female 172(30.3%),319(56.3%),in gradeⅠ,204(36.0%)in gradeⅡ,38(6.7%)in gradeⅢ,and 6(1.1%)in gradeⅣ.Single factor linear regression analysis showed that Age,gender and COPD grade were the factors affecting the quality of life of the elderly,with statistical difference(P<0.05).The analysis among different grading groups showed that there was no significant difference in the quality of life between gradeⅠand gradeⅡpatients(P>0.05),but there was statistical difference in the quality of life between gradeⅠ,gradeⅡ,gradeⅢand gradeⅣpatients(P<0.05).Conclusion Age,gender and COPD grade are the factors affecting the quality of life in the elderly patients with COPD,and they are negatively correlated with grade degree.
