摘  要:目的探讨正常听力青年人言语声诱发听性脑干反应(speech-evoked auditory brainstem response,s-ABR)以及扩展高频测听的性别、年龄差异,旨在提供不同性别、年龄正常听力人群的参考数据。方法招募171例(171耳)18~42周岁纯音听阈正常、无噪声暴露史的正常听力青年人,其中,女82耳,男89耳;18~30岁(大于18岁小于等于30岁)组96耳(男46耳,女50耳),30~42岁(大于30小于等于42岁)组54耳(男、女各27耳),分别进行常频及扩展高频测听、s-ABR测试,比较s-ABR各波潜伏期和幅值以及各频率阈值的差异,分析s-ABR各波潜伏期、幅值以及V/A复合体与常频及扩展高频纯音测听的相关性。结果性别比较:①女性s-ABR的波V、C和F潜伏期显著短于男性(P<0.05),V/A幅值显著高于男性(P<0.05);②除4 kHz外,其余各频率平均听阈无明显性别差异(P>0.05)。年龄比较:①18~30岁组波O幅值显著高于30~42岁组(P<0.05),V/A斜率显著大于30~42岁组;②除0.25 kHz外,其余各频率平均听阈值18~30岁组显著低于30~42岁组(P<0.05)。相关性分析:波O幅值与PTA_(0.5~4)、PTA_(3~8)和PTA EHF呈正相关(P<0.05);波A潜伏期、V/A波间期与PTA_(3~8)呈正相关(P<0.05);V/A斜率与PTA_(3~8)、PTA EHF呈负相关(P<0.05)。结论性别和年龄因素对脑干和外周听觉系统对言语声刺激的反应有不同程度的影响,高频听力对言语声精细结构判别上起作用。Objective To investigate the sex and age differences in speech-evoked auditory brainstem response(s-ABR)and extended high-frequency audiometry in young adults with normal hearing,and to provide reference data for normal hearing population of different gender and age groups.Methods A total of 171 young adults(171 ears)aged 18~42 years old with normal hearing threshold and no noise exposure history were recruited and divided into different gender groups(82 ears for females,89 ears for males)and age groups(18~30 years old group and 30~42 years old group).The conventional frequency,extended high-frequency audiometry and s-ABR tests were conducted.The differences of the latency and amplitude of each wave of s-ABR and the threshold of each frequency were compared.The correlation between the latency,amplitude of each wave,and V/A complex of s-ABR and the conventional frequency and extended high-frequency pure tone audiometry was analyzed.Results Gender comparison:①The latencies of V,C and F waves of s-ABR in female were significantly shorter than those in male(P<0.05).The amplitudes of V/A in female were significantly higher than those in male(P<0.05).②Except for 4 kHz,there was no significant gender difference in the average hearing threshold of frequencies(P>0.05).Age comparison:①The amplitude of O wave in the 18~30 years old group was significantly higher than that in the 30~42 years old group(P<0.05),and the slope of V/A was significantly bigger than that in the 30~42 years old group.②Except 0.25 kHz,the average hearing threshold of other frequencies in the 18~30 years old group was significantly lower than that in the 30~42 years old group(P<0.05).Correlation analysis results:O wave amplitude was positively correlated with PTA_(0.5~4),PTA_(3~8)and PTA EHF(P<0.05).The latency of A wave and V/A wave interval were positively correlated with PTA_(3~8)(P<0.05).The V/A slope was negatively correlated with PTA_(3~8)and PTA EHF(P<0.05).Conclusion Gender and age factors have different effects on the response of brai
