摘　　要：在国家基本公共卫生服务项目的填报过程中,规范填报对项目实施效果具有一定的提升作用。基于某市的填报及实施情况,分析全市社区卫生服务中心国家基本公共卫生报表数据,梳理国家基本公共卫生指标及规范填报要求,统计报表填报情况、区级考核规范达标情况、社区卫生服务中心考核规范未达标情况。发现需要通过规范电子健康档案质控标准及数据来源,开展区级质控、建设健康管理信息化平台及一体化管理中心等来达到老年人健康管理率的绩效指标。社区卫生服务中心居民对高血压患者管理、糖尿病患者管理、健康档案管理、65岁以上老年人体检有很大的需求,可引入市疾病预防控制中心牵头的标准化测量系统来改善社区卫生服务中心慢性病管理不规范的问题。In the process of filling in the national basic public health service project,the standard filling has a certain effect on the implementation of the project.Based on the filling and implementation of a city,the data of the national basic public health report of the communities in the city is analyzed;the national basic public health indicators and standard filling requirements are summarized;and statistics on the filling situation of the report,the meeting of the district-level assessment standards,the failing of the community assessment standards were done.It is found that it is necessary to standardize the quality control standards and data sources of electronic health records,carry out district-level quality control,build health management information platform and integrated management center to achieve the performance indicators of the elderly health management rate.Residents in the community have great demands for the management of hypertension patients,diabetes patients,health records management,and physical examination of elderly people over 65 years old.The standardized measurement system led by the municipal center for disease control and prevention can be introduced to improve the irregular management of chronic diseases in the community.
