摘  要:目的分析“探索提高贫困地区肺结核发现水平”项目实施效果,为探讨在临汾市建立区域性肺结核诊疗中心的可行性提供科学依据。方法利用中国疾病预防控制信息系统及其子系统“结核病管理信息系统”有关“探索提高贫困地区肺结核发现水平”项目(以下简称项目)月报表的相关数据,对项目实施前2年(2017—2018年)和项目实施后2年(2019—2020年)初诊患者、活动性肺结核患者、病原学阳性患者、利福平耐药肺结核患者登记等情况进行分析。结果活动性肺结核患者登记率由项目实施前的47.93/10万上升至项目实施后的59.57/10万,差异无统计学意义(χ^(2)=3.355,P=0.067);病原学阳性率由项目实施前的26.77%上升至项目实施后的42.14%,差异有统计学意义(χ^(2)=7.87,P=0.005)。项目实施后发现4例利福平耐药肺结核患者。结论通过项目实施,建立了一套行之有效的活动性肺结核和利福平耐药肺结核分子生物学诊断流程,有效地提高了活动性肺结核患者发现水平和病原学阳性率,为探讨在临汾市建立区域性肺结核诊疗中心的可行性提供科学依据。Objective To analyze the implementation effect of“Exploring and Improving the Level of Tuberculosis Detection in Poor Areas”Project,and to provide scientific basis for establishing regional diagnosis and treatment center in Linfen City.Methods Relevant data were collected from China's Disease Prevention and Control Information System and its subsystem“TB Management Information System”,and the monthly report of the“Exploring and Improving the Level of Tuberculosis Detection in Poor Areas”Project.The registration of newly diagnosed patients,active tuberculosis patients,etiologically positive patients and patients with rifampicin drug-resistant tuberculosis in the two years before project(from 2017 to 2018)and the two years after project(from 2019 to 2020)were analyzed.Results The registration rate of active tuberculosis patients increased from 47.93/100000 before implementation to 59.57/100000 after implementation,with no statistical significance(χ^(2)=3.355,P=0.067).The positive rate of pathogens increased from 26.77% before implementation to 42.14% after implementation,with statistically significant difference(χ^(2)=7.87,P=0.005).After implementation of the project,4 patients with rifampicin resistant pulmonary tuberculosis were found.Conclusion A set of effective molecular biological diagnosis process for active pulmonary tuberculosis and rifampicin resistant pulmonary tuberculosis has been established,which effectively improves the detection level and ecdogical positive rate of patients with active pulmonary tuberculosis,provides a scientific basis for exploring the establishment of regional diagnosis and treatment centers in Linfen City.
