湿润烧伤膏联合银离子敷料对Ⅲ~Ⅳ期压力性损伤的效果及其机体炎症、Caspase-3蛋白/VEGF水平的影响

摘　　要：目的:探究湿润烧伤膏(moist exposed burn ointment,MEBO)联合银离子敷料对Ⅲ~Ⅳ期压力性损伤(pressure ulcer,PU)患者的治疗疗效及对机体炎症指标、Caspase-3蛋白/血管内皮生长因子(vascular endothelial growth factor,VEGF)水平的影响。方法:2020年1月至2022年5月在海南省第五人民医院皮肤康复科接受治疗的Ⅲ~Ⅳ期PU患者中随机选取126例,采用随机数表法分为观察组和对照组,患者均为63例。所有患者均行常规治疗措施,同时对照组给予银离子敷料治疗,观察组在对照组基础上联合MEBO治疗。对比2组患者治疗疗效、治疗进度,以及治疗前后血清指标的变化情况。结果:观察组患者的治疗总有效率高于对照组,同时创面愈合时间短于对照组,差异具有统计学意义(P<0.05)。治疗2周后,观察组血清肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(interleukin-6,IL-6)以及C反应蛋白(C-reactive protein,CRP)低于对照组,同时VEGF、Caspase-3蛋白高于对照组,差异均有统计学意义(均P<0.05)。结论:Ⅲ~Ⅳ期PU患者采用MEBO联合银离子敷料治疗,疗效显著优于单纯银离子敷料治疗。联合治疗还能显著缓解机体炎症,促进组织血管新生,缩短愈合时间。Objective:To explore the effects of moist exposed burn ointment(MEBO)combined with silver ion dressing on patients with stagesⅢ−Ⅳstress injury(PU)and the influence of inflammation index and Caspase-3 protein/vascular endothelial growth factor(VEGF)level.Methods:A total of 126 patients with stages III−IV PU were randomly selected from the Department of Skin Rehabilitation of the Fifth People’s Hospital of Hainan Province from January 2020 to May 2022,who were divided into an observation group and a control group by random number table method,each group with 63 patients.All patients received conventional treatment,while the control group was treated with silver ion dressing,and the observation group was treated with MEBO on the basis of the control group.The therapeutic effect,treatment schedule and cost in the 2 groups were compared,and the changes of serum indexes before and after treatment were compared.Results:The total effective rate was higher,and the wound healing time was shorter in the observation group than that in the control group,there was significant difference(P<0.05).After 2 weeks of treatment,the serum tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and C-reactive protein(CRP)were lower in the observation group than those in the control group(all P<0.05).At the same time,VEGF and Caspase-3 proteins were higher in the observation group than those in the control group,there were significant difference(all P<0.05).Conclusion:The treatment effect of MEBO combined with silver ion dressing is significantly better than that of silver ion dressing alone in patients with stageⅢtoⅣPU.Combined therapy can also significantly relieve inflammation,promote angiogenesis,and shorten healing time.
