摘 要:目的观察不同肠内营养方式对重症监护室(ICU)患者发生呼吸机相关性肺炎方面的疗效。方法选取江西省新余市人民医院2020年1月1日-2021年12月31日期间于ICU行机械通气且治疗时间大于48小时的80例患者,依据随机数字表法分为鼻胃管组(利用常规插管法把鼻胃管末端置于胃部)、鼻肠管组(利用盲插法把鼻肠管末端置于十二指肠降部),每组40例,对比两组干预效果。结果相比鼻胃管组,鼻肠管组机械通气和住ICU时间更短,不良反应和呼吸机相关性肺炎发生率更低,营养指标水平更高(P<0.05)。结论鼻肠管肠内营养方式用在ICU危重患者中可获得更好的疗效,预后效果理想。Objective To observe the effect of different enteral nutrition modalities in the occurrence of ventilator-associated pneumonia in intensive care unit(ICU)patients.Methods Eighty patients whose treatment time was higher than 48 hours from ICU admission to the people's Hospital of Xinyu City,Jiangxi,people's Hospital 2020.1.1-2021.12.31,were selected and divided according to the random number table into a nasogastric group(using conventional intubation to place the end of the nasogastric tube in the stomach),a nasogastric group(using a blind insertion to place the end of the nasogastric tube in the descending duodenum),and 40 patients in each group,to compare the intervention effects between the two groups.Results Compared with the nasogastric group,the Nasoenteric group had a shorter incidence of mechanical ventilation and ICU hospitalization time,fewer adverse effects and ventilator-associated pneumonia,and higher levels of nutritional indicators(P<0.05).Conclusions Nasoenteric enteral nutrition modality use in critically ill patients in the ICU can achieve better efficacy with ideal prognostic results.
