针刺治疗子宫腺肌症痛经的临床观察

摘　　要：目的观察针刺治疗子宫腺肌症痛经症状的疗效及安全性。方法将60例子宫腺肌症伴痛经的患者随机分为观察组和对照组,每组各30例。观察组采用调经促孕针法针刺,月经来潮前7天开始针刺,直至经期痛经缓解时结束针刺治疗,每日针刺1次。对照组口服布洛芬缓释胶囊镇痛,每次0.3g,每日2次,痛经显著缓解时停止服用。两组均治疗3个月经周期。观察两组患者VAS评分、CMSS量表评分、有效率和安全性评价。结果第3个月经周期后,观察组患者VAS得分显著低于对照组(P<0.01)。第2个月经周期治疗和第3个月经周期治疗后,两组CMSS量表得分无统计学差异(P>0.05)。观察组总有效率为100%,对照组总有效率为93.33%,两组间无统计学差异(P>0.05)。结论针刺明显缓解了子宫腺肌症患者痛经程度和持续时间,等于甚至优于布洛芬的镇痛疗效,并且对痛经伴随症状,如恶心呕吐、焦虑抑郁、背痛、失眠等也有很好的疗效。Objective To observe the efficacy and safety of acupuncture in the treatment of dysmenorrhea symptoms in adenomyosis.Methods Sixty patients with adenomyosis with dysmenorrhea were randomly divided into an observation group and a control group,with 30 cases in each group.In the observation group,acupuncture was used and the treatment of acupuncture was started 7 days before the onset of menstruation until the end of acupuncture treatment when dysmenorrhea was relieved during menstruation,and acupuncture was performed once a day.The control group received oral analgesia with ibuprofen extended-release capsule,0.3g each time,twice a day,and stopped when the dysmenorrhea was significantly relieved.Both groups were treated for 3 menstrual cycles.The VAS score and CMSS scale score and the efficiency and safety evaluation were observed in both groups.Results The VAS scores of patients in the observation group were significantly lower than those in the control group after the 3rd menstrual cycle(P<0.01).There was no statistical difference between the CMSS scale scores of the two groups after the 2nd menstrual cycle treatment and the 3rd menstrual cycle treatment(P>0.05).The total effective rate was 100%in the observation group and 93.33%in the control group,with no statistical difference between the two groups(P>0.05).Conclusion Acupuncture significantly relieved the degree and duration of dysmenorrhea in patients with adenomyosis,which was equal to or even better than the analgesic efficacy of ibuprofen,and also had good efficacy on concomitant symptoms such as nausea and vomiting,anxiety and depression,back pain,insomnia and other concomitant symptoms of dysmenorrhea.
