摘 要:目的总结日照市泌尿系结石患者的结石化学成分,为临床防治泌尿系结石疾病提供帮助。方法收集日照市中医医院2019年12月—2021年12月住院患者手术取出的结石标本及体外冲击波碎石排出的结石标本共643例,以红外光谱法分析结石标本的组成成分,对不同年龄、性别患者的结石组成成分进行分析总结。结果本组643例结石标本中单纯性结石占51.48%(331/643),混合性结石占48.52%(312/643);结石成分分析中,草酸钙的检出率(占77.92%)最高,碳酸磷灰石的检出率(占36.39%)为第2位,检出率为第3位的是尿酸结石(占28.62%),其他成分结石所占比例较低。尿路结石患者男、女之比为2.12∶1,二者结石成分主要是草酸钙、碳酸磷灰石、尿酸。随着年龄增长,泌尿系结石的发生率逐渐升高,40~59岁时达高峰。结论山东省日照市患者的尿路结石化学成分以草酸钙为主,告知患者及家属尿路结石的成分,对于指导治疗、明确结石发生的原因以及生活中预防复发有重要的社会意义。Objective To summarize the chemical composition of stones in patients with urinary stones in Rizhao to provide clinical assistance in the prevention and treatment of urinary stone disease.Methods A total of 643 stone specimens surgically removed from inpatients and stone specimens discharged by extracorporeal shock wave lithotripsy were collected from December 2019 to December 2021 in Rizhao Traditional Chinese Medicine Hospital,and the composition of stone specimens was analyzed by infrared spectroscopy to summarize the composition of stones in patients of different ages and genders.Results Among the 643 stone specimens in this group,simple stones accounted for 51.48%(331/643)and mixed stones accounted for 48.52%(312/643),in the analysis of stone composition,the detection rate of calcium oxalate(77.92%)was the highest,the detection rate of apatite carbonate(36.39%)was the second,and the detection rate of uric acid stones(28.62%)was the third,and other the percentage of other components of stones was low.The ratio of men to women with urinary stones was 2.12:1,and the main components of both stones were calcium oxalate,carbonate apatite,and uric acid.The incidence of urinary stones gradually increased with age,reaching a peak at the age of 40-59 years.Conclusion The chemical composition of urinary stones in patients in Rizhao,Shandong Province,is mainly calcium oxalate.Informing patients and their families of the composition of urinary stones is socially important for guiding treatment,clarifying the causes of stone occurrence,and preventing recurrence in life.
