耳尖放血联合祛湿止痒方治疗黄疸性皮肤瘙痒的临床研究

摘　　要：目的观察耳尖放血联合祛湿止痒方治疗黄疸性皮肤瘙痒的临床效果。方法选择青岛市黄岛区中医医院2022年2月—2023年2月因黄疸病住院治疗且伴随皮肤瘙痒症状的120例急慢性黄疸型肝炎患者,随机分为治疗组与对照组,每组各60例。对照组采取常规保肝等对症治疗,治疗组在对照组基础上给予耳尖放血联合中药祛湿止痒方治疗,2周为1个疗程。观察并比较两组患者临床疗效,治疗前后黄疸消退时间、皮肤瘙消退时间及血清谷草转氨酶(glutamic oxaloacetic transaminase,AST)、谷丙转氨酶(alanine aminotransferase,ALT)、直接胆红素(direct bilirubin,DBIL)、总胆红素(total bilirubin,TBIL)等。结果治疗组和对照组总有效率分别为91.67%、60.00%,治疗组优于对照组(P<0.05);治疗组治疗后黄疸消退时间、皮肤瘙痒消退时间均短于对照组(P<0.05);治疗后两组血清学ALT、AST、TBIL、DBIL均明显降低,治疗组优于对照组(P<0.05)。结论耳尖放血联合祛湿止痒方治疗黄疸性皮肤瘙痒疗效显著,可明显缓解患者症状,缩短病程。Objective To observe the clinical effect of ear tip bleeding combined with Qushi Zhiyang prescription in the treatment of icteric skin pruritus.Methods A total of 120 patients with acute and chronic icteric hepatitis who were hospitalized for jaundice and accompanied with skin pruritus from February 2022 to February 2023 in Traditional Chinese Medicine Hospital of Huangdao District,Qingdao were selected and randomly divided into treatment group and control group,with 60 cases in each group.The control group was treated with routine liver protection and other symptomatic treatment,and the treatment group was treated with ear tip bleeding combined with traditional Chinese medicine for removing dampness and relieving itching on the basis of the control group,with a course of treatment of 2 weeks.To observe and compare the clinical efficacy between the two groups,which only owns jaundice resolution time,skin antimicrobial time and serum glutamic oxaloacetic transaminase(AST)and alanine aminotransferase(ALT),direct bilirubin(DBIL),total bilirubin(TBIL)before and after treatment.Results The total effective rate of the treatment group and the control group was 91.67%and 60.00%,respectively,and the treatment group was better than the control group(P<0.05).After treatment,the time for jaundice and skin itching in the treatment group were significantly shorter than those in the control group(P<0.05).After treatment,serum ALT,AST,TBIL and DBIL were significantly decreased in both groups,the treatment group was better than the control group(P<0.05).Conclusion The clinical effect of ear tip bleeding combined with Qushi Zhiyang prescription in the treatment of icteric skin pruritus is definite,which can obviously relieve the clinical symptoms of patients and shorten the course of disease.
