螺钉固定治疗Lauge-Hansen旋后-内收型Ⅱ度踝关节骨折的效果分析

摘　　要：目的探究螺钉固定在旋后-内收型Ⅱ度踝关节骨折中的应用效果。方法选择中国人民解放军第九〇七医院2019年1月—2020年5月收治的78例踝关节骨折(旋后-内收型Ⅱ度)患者,运用计算机软件分为2组,A组(n=39)采用支撑钢板固定,B组(n=39)采用螺钉固定,观察并比较2组负重时间、踝关节活动度、骨折愈合时间、并发症及Kofoed和美国足踝外科协会(american orthopedic foot and ankle society,AOFAS)评分。结果A组负重时间(部分负重和完全负重时间)短于B组(P<0.05);A组踝关节活动度大于B组(P<0.05);2组骨折愈合时间、总并发症发生率比较差异无统计学意义(P>0.05);治疗后2组踝关节Kofoed评分、AOFAS评分均增高,且A组高于B组(P<0.05)。结论对旋后-内收型Ⅱ度踝关节骨折患者运用螺钉固定或支撑钢板固定效果都较好,但后者对促进患者早日负重和功能恢复方面优势更为明显。Objective To explore the application of screw fixation in patients with supination adduction typeⅡankle fracture.Methods 78 patients with internal rotation fracture of ankle in the people's Liberation Army hospital from January 2019 to May 2020 were strictly selected.The patients were divided into two groups by computer software.Group A(n=39)was fixed with supporting plate and group B(n=39)was fixed with screw.During the follow-up,the weight-bearing time,ankle range of motion,fracture healing time,complications and kofoed and AOFAS scores were observed and compared between the two groups.Results The weight-bearing time(partial and complete weight-bearing time)in group A was shorter than that in group B(P<0.05).The range of motion of ankle joint in group A was greater than that in group B(P<0.05).There was no significant difference in fracture healing time and total complication rate between the two groups(P>0.05).After treatment,the ankle kofoed score and AOFAS score of the two groups increased(P<0.05),and the score of group A was higher than that of group B(P<0.05).Conclusion The effect of screw fixation or supporting plate fixation in patients with supination adduction typeⅡankle fracture is better,but the latter has more obvious advantages in promoting early weight-bearing and functional recovery.
