腹腔镜完全腹膜外腹股沟疝修补术与充填式无张力腹股沟疝修补术疗效对比

摘　　要：目的比较腹腔镜完全腹膜外腹股沟疝修补术与疝环充填式无张力疝修补术治疗腹股沟疝的临床疗效。方法回顾性分析2017年12月—2019年12月兴宁市人民医院收治的348例腹股沟疝患者,按手术方式不同分为腹腔镜手术组和开放手术组,每组174例。腹腔镜手术组行腹腔镜完全腹膜外疝修补术,开放手术组行充填式无张力腹股沟疝修补术,对比2组患者的手术时间、住院费用、住院时间、舒适指数评分(bruggrmann comfort scale,BCS)、术后下床活动时间、并发症发生率等。结果与开放手术组相比,腹腔镜手术组患者的手术时间短、住院时间短、BCS评分较高、术后下床活动时间快,但住院费用较高,差异有统计学意义(P<0.05)。腹腔镜手术组术后并发症发生率低于开放手术组,差异有统计学意义(P<0.05)。结论与疝环充填式无张力疝修补术相比,腹腔镜完全腹膜外修补术治疗腹股沟疝具有并发症发生少、舒适程度高、术后下床活动时间快、住院时间短等优点,值得推广。Objective The aim of this study was to compare the clinical efficacy of laparoscopic total extraperitoneal inguinal hernia repair and filling tension-free hernia repair.Methods The study utilised a comparison control group design with two groups of laparoscopic surgery group and open surgery group.A retrospective analysis of 348 patients with inguinal hernia admitted to our department from December 2017 to December 2019 were divided into two groups,174 cases in each group.Laparoscopic surgery group was underwent laparoscopic total extraperitoneal inguinal hernia repair.In the open surgery group,a tension-free inguinal hernia repair was performed.The operation time,hospitalization cost,hospitalization time,comfort index score BCS,postoperative activity time,and incidence of postoperative complications were compared between the two groups.Results Compared with the open surgery group,patients in the laparoscopic surgery group had shorter operation time,shorter hospital stay,higher BCS comfort index score,and faster time to get out of bed after surgery.The difference between the two groups was statistically significant(P<0.05).However,the cost of hospitalization in the laparoscopic surgery group was higher.The incidence of postoperative complications in the laparoscopic surgery group was lower than that in the open surgery group,and the difference was statistically significant(P<0.05).Conclusion Compared with filling tension-free inguinal hernia repair,laparoscopic total extraperitoneal repair for inguinal hernia has the advantages of less complications,higher comfort,quicker time to get out of bed after surgery,and shorter hospital stay.It is worthy of clinical application and promotion in primary hospitals.
