摘 要:目的分析多索茶碱注射液联合无创正压通气治疗慢阻肺合并2型呼吸衰竭的临床效果。方法选取2018年1月至2020年3月接收的90例慢阻肺合并2型呼吸衰竭患者作为研究对象,根据治疗方案的不同将其分为对照组(45例,无创正压通气治疗)和观察组(45例,多索茶碱注射液联合无创正压通气治疗)。比较两组的治疗效果。结果治疗后,观察组的Clara细胞分泌蛋白16(CC16)水平高于对照组,降钙素原(PCT)、巨噬细胞刺激蛋白(MSP)及白细胞介素-17(IL-17)水平低于对照组(P<0.05)。治疗后,观察组的用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、FEV_(1)/FVC及每分钟最大通气量(MVV)高于对照组(P<0.05)。随访后,观察组的慢阻肺评估测试(CAT)、圣乔治呼吸问卷(SGRQ)评分低于对照组(P<0.05)。观察组的治疗总有效率高于对照组(P<0.05)。结论多索茶碱注射液联合无创正压通气治疗慢阻肺合并2型呼吸衰竭能有效缓解患者的病情,改善血清生化因子水平,提高肺功能。Objective To analyze the clinical effect of doxofylline injection combined with noninvasive positive pressure ventilation in the treatment of chronic obstructive pulmonary disease complicated with type 2 respiratory failure.Methods A total of 90 patients with chronic obstructive pulmonary disease complicated with type 2 respiratory failure received from January 2018 to March 2020 were selected as the research objects.According to different treatment schemes,the patients were divided into control group(45 cases,noninvasive positive pressure ventilation treatment)and observation group(45 cases,doxofylline injection combined with noninvasive positive pressure ventilation treatment).The therapeutic effects of the two groups were compared.Results After treatment,the level of Clara cell secretory protein 16(CC16)in the observation group was higher than that in the control group,and the levels of procalcitonin(PCT),macrophage stimulating protein(MSP)and interleukin-17(IL-17)were lower than those in the control group(P<0.05).After treatment,the forced vital capacity(FVC),forced expiratory volume in one second(FEV_(1)),FEV_(1)/FVC and maximum ventilation volume(MVV)in the observation group were higher than those in the control group(P<0.05).After follow-up,the scores of COPD Assessment Test(CAT)and St.George's Respiratory Questionnaire(SGRQ)of the observation group were lower than those in the control group(P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).Conclusion Doxofylline injection combined with noninvasive positive pressure ventilation in the treatment of chronic obstructive pulmonary disease complicated with type 2 respiratory failure can effectively alleviate the patients'condition,improve the levels of serum biochemical factors,and improve lung function.
