后路长节段与短节段椎弓根钉棒系统固定术治疗退变性脊柱侧弯的效果及对JOA、ODI评分的影响

摘　　要：目的探讨后路长节段与短节段椎弓根钉棒系统固定术治疗退变性脊柱侧弯的效果。方法选取2019年1月至2021年12月我院收治的68例退变性脊柱侧弯患者为研究对象,按照随机数字表法将其分为长节段组(后路长节段椎弓根钉棒系统固定术)和短节段组(后路短节段椎弓根钉棒系统固定术),各34例。比较两组的治疗效果。结果长节段组的手术时间长于短节段组,术中出血量多于短节段组,差异具有统计学意义(P<0.05);两组的术后下床活动时间及住院时间比较,差异无统计学意义(P>0.05)。术后1个月,两组的Cobb角均减小,腰椎前凸角度均增大,且长节段组优于短节段组,差异具有统计学意义(P<0.05)。术后1个月,两组的日本骨科协会(JOA)评分均升高,Oswestry功能障碍指数(ODI)评分均降低,且长节段组优于短节段组,差异具有统计学意义(P<0.05)。两组的不良事件总发生率比较,差异无统计学意义(P>0.05)。结论后路长节段与短节段椎弓根钉棒系统固定术治疗退变性脊柱侧弯各有优劣势,长节段椎弓根钉棒系统固定术能够提高脊柱功能,改善Cobb角和腰椎前凸角度,而短节段椎弓根钉棒系统固定术能够缩短手术时间,减少术中出血量,这就需要根据患者具体情况酌情选择。Objective To explore the effect of posterior long segment and short segment pedicle screw rod system fixation in the treatment of degenerative scoliosis.Methods A total of 68 patients with degenerative scoliosis admitted in our hospital from January 2019 to December 2021 were selected as the research objects.According to the random number table method,the patients were divided into long segment group(posterior long segment pedicle screw rod system fixation)and short segment group(posterior short segment pedicle screw rod system fixation),with 34 cases in each group.The therapeutic effects of the two groups were compared.Results The operation time of the long segment group was longer than that of the short segment group,and the intraoperative blood loss was more than that of the short segment group,the differences were statistically significant(P<0.05);there were no significant differences in postoperative ambulation time and hospitalization time between the two groups(P>0.05).One month after operation,the Cobb angle of the two groups decreased,the lumbar lordosis angle increased,and those in the long segment group were better than the short segment group,the differences were statistically significant(P<0.05).One month after operation,the Japanese Orthopaedic Association(JOA)score of the two groups increased,the Oswestry Disability Index(ODI)score decreased,and those in the long segment group were better than the short segment group,the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse events between the two groups(P>0.05).Conclusion Posterior long segment and short segment pedicle screw rod system fixation have their own advantages and disadvantages in the treatment of degenerative scoliosis.Long segment pedicle screw rod system fixation can improve spinal function,Cobb angle and lumbar lordosis angle,while short segment pedicle screw rod system fixation can shorten operation time and reduce intraoperative blood loss,which should be selected a
