鼻内镜下低温等离子射频消融术对鼻前庭囊肿患者预后的影响

摘　　要：目的分析鼻内镜下低温等离子射频消融术对鼻前庭囊肿患者预后的影响。方法选取2018年5月至2021年5月我院收治的80例鼻前庭囊肿患者作为研究对象,根据治疗方案的不同将其分为对照组和观察组,各40例。对照组给予鼻内镜下鼻前庭囊肿揭盖术治疗,观察组给予鼻内镜下低温等离子射频消融术治疗。比较两组的手术及预后相关指标、疼痛情况、并发症发生情况以及炎性因子水平。结果观察组的手术时间、伤口愈合时间、术腔上皮化时间均短于对照组,术中出血量少于对照组,差异具有统计学意义(P<0.05)。术后6、12、24 h,观察组的视觉模拟评分法(VAS)评分均低于对照组,差异具有统计学意义(P<0.05)。观察组的术后并发症总发生率为7.50%,低于对照组的25.00%,差异具有统计学意义(P<0.05)。术后3 d,观察组的C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)及白细胞介素-8(IL-8)水平明显低于对照组,差异具有统计学意义(P<0.05)。结论鼻内镜下低温等离子射频消融术能有效缩短鼻前庭囊肿患者的手术时间、减少术中出血量,促进创口愈合,且术后患者疼痛度较轻、并发症发生率较低、炎性因子水平下降快,有利于患者的预后。Objective To analyze the effect of low temperature plasma radiofrequency ablation under nasal endoscope on the prognosis of patients with nasal vestibular cyst.Methods A total of 80 patients with nasal vestibular cyst admitted in our hospital from May 2018 to May 2021 were selected as the research objects and divided into control group and observation group according to different treatment schemes,with 40 cases in each group.The control group was treated with nasal vestibular cyst uncovering under nasal endoscope,and the observation group was treated with low temperature plasma radiofrequency ablation under nasal endoscope.The surgical and prognostic related indicators,pain,complications and inflammatory factors levels were compared between the two groups.Results The operation time,wound healing time and epithelization time of surgical cavity in the observation group were shorter than those in the control group,the intraoperative blood loss was less than that in the control group,and the differences were statistically significant(P<0.05).At 6,12 and 24 h after operation,the Visual Analogue scale(VAS)scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).The total incidence of postoperative complications in the observation group was 7.50%,which was lower than 25.00%in the control group,and the difference was statistically significant(P<0.05).At 3 d after operation,the levels of C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and interleukin-8(IL-8)in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Low temperature plasma radiofrequency ablation under nasal endoscope can effectively shorten the operation time of patients with nasal vestibular cyst,reduce the amount of intraoperative blood loss,and promote wound healing.Moreover,the postoperative pain of patients is mild,the incidence of complications is low,and
