能谱CT最佳单能量成像优化腹腔干动脉图像质量的临床价值

摘  要:目的探讨能谱计算机断层扫描(CT)最佳单能量成像优化腹腔干动脉图像质量的临床价值。方法将60例行腹腔干动脉计算机断层血管成像(CTA)的患者随机分为A组和B组,各30例。A组行常规120 kVp扫描,对比剂用量600 mgI/kg,扫描后重建40%自适应统计迭代重建(ASIR)图像;B组行能谱CT扫描,对比剂用量450 mgI/kg,扫描后重建最佳单能量、40%ASIR图像。比较两组患者的辐射剂量和对比剂碘摄入量;比较两组患者的腹腔干动脉CT、标准差(SD)、信噪比(SNR)、对比噪声比(CNR)值和影像医生主观评分。结果B组的剂量长度乘积、有效辐射剂量、对比剂碘摄入量低于A组(P<0.05);B组的有效辐射剂量和对比剂碘摄入量较A组分别减少约20.29%和28.64%。B组的腹腔干动脉最佳单能量约(65.78±3.15)keV。B组的腹腔干动脉CT、SNR、CNR值及影像医生主观评分高于A组,腹腔干动脉SD值低于A组(P<0.05)。2名影像医生对腹腔干血管主观评分具有很好一致性(Kappa>0.90,P<0.05)。结论与常规120 kVp腹腔干动脉成像相比,能谱CT最佳单能量成像不仅可以降低辐射剂量和对比剂碘摄入量,还可以显著提升图像质量。Objective To explore the clinical value of optimum single energy imaging of spectral computed tomography(CT)in optimizing the image quality of coeliac trunk artery.Methods Sixty patients who underwent computed tomography angiography(CTA)of coeliac trunk artery were randomly divided into group A and group B,with 30 cases in each group.In the group A,conventional 120 kVp scan was performed,the amount of contrast agent was 600 mgI/kg,and 40%adaptive statistical iterative reconstruction(ASIR)image was reconstructed after scanning;in the group B,spectral CT scan was performed with a dose of 450 mgI/kg contrast agent,and the best single energy and 40%ASIR image was reconstructed after scanning.The radiation dose and contrast agent iodine intake were compared between the two groups;the coeliac trunk artery CT,standard deviation(SD),signal to noise ratio(SNR),contrast to noise ratio(CNR)and radiologists'subjective scores were compared between the two groups.Results The dose length product,effective radiation dose and contrast agent iodine intake in the group B were lower than those in the group A(P<0.05).The effective radiation dose and contrast agent iodine intake in the group B decreased by approximately 20.29%and 28.64%compared with the group A,respectively.The optimum single energy of coeliac trunk artery in the group B was about(65.78±3.15)keV.The CT,SNR and CNR values of coeliac trunk vessel and the subjective score of radiologists in the group B were higher than those in the group A,and the SD value of coeliac trunk artery was lower than that in the group A(P<0.05).The two radiologists had good consistency in the subjective scores of coeliac trunk artery(Kappa>0.90,P<0.05).Conclusion Compared with conventional 120 kVp coeliac trunk artery imaging,spectral CT optimum single energy imaging can not only reduce radiation dose and contrast agent iodine intake,but also significantly improve image quality.
