摘  要:目的探讨低剂量冠状动脉计算机断层血管造影(CTA)在心肌桥-壁冠状动脉血管形态学诊断中的应用价值。方法选择2019年8月至2020年4月于河南省南阳市第一人民医院诊治的320例冠心病患者作为研究对象,根据随机数字表法将其分为低剂量组与对照组,各160例。低剂量组给予低剂量冠状动脉CTA扫描,对照组给予常规剂量冠状动脉CTA扫描。比较两组的影像学特征。结果两组的图像质量评分均>3分,组间比较,差异无统计学意义(P>0.05)。低剂量组的容积CT剂量指数(CTDIvol)、剂量长度乘积(DLP)、有效辐射剂量(ED)均显著低于对照组,差异具有统计学意义(P<0.05)。低剂量组检出心肌桥-壁冠状动脉45例,其中浅表型34例,深在型11例;对照组检出心肌桥-壁冠状动脉44例,其中浅表型33例,深在型11例。两组的心肌桥-壁冠状动脉检出率比较,差异无统计学意义(P>0.05)。在低剂量组中,心肌桥-壁冠状动脉分布于前降支37例,对角支1例,回旋支1例,钝缘支3例,右冠状动脉3例;在对照组中,心肌桥-壁冠状动脉分布于前降支33例,对角支2例,回旋支1例,钝缘支4例,右冠状动脉4例。结论低剂量冠状动脉CTA在心肌桥-壁冠状动脉血管形态学诊断中的应用价值显著,可在保证图像质量的基础上降低辐射剂量,值得临床大力推广应用。Objective To investigate the application value of low-dose coronary computed tomography angiography(CTA)in the vascular morphological diagnosis of myocardial bridge-mural coronary artery.Methods A total of 320 patients with coronary heart disease diagnosed and treated in Nanyang First People's Hospital from August 2019 to April 2020 were selected as the research objects.According to the random number table method,the patients were divided into low-dose group and control group,with 160 cases in each group.The low-dose group was given low dose coronary CTA scan,and the control group was given conventional dose coronary CTA scan.The imaging features of the two groups were compared.Results The image quality score of the two groups was>3 points,and there was no significant difference between the two groups(P>0.05).The CT dose index of volume(CTDIvol),dose length product(DLP)and effective dose(ED)of the low-dose group were significantly lower than those of the control group,and the differences were statistically significant(P<0.05).In the low-dose group,45 cases of myocardial bridge-mural coronary artery were detected,including 34 cases of superficial type and 11 cases of deep type;in the control group,44 cases of myocardial bridge-mural coronary artery were detected,including 33 cases of superficial type and 11 cases of deep type.There was no significant difference in the detection rate of myocardial bridge-mural coronary artery between the two groups(P>0.05).In the low-dose group,myocardial bridge-mural coronary artery was distributed in 37 cases of anterior descending branch,1 case of diagonal branch,1 case of circumflex branch,3 cases of blunt marginal branch,and 3 cases of right coronary artery;in the control group,the myocardial bridge-mural coronary artery was distributed in 33 cases of anterior descending branch,2 cases of diagonal branch,1 case of circumflex branch,4 cases of blunt marginal branch,and 4 cases of right coronary artery.Conclusion The low-dose coronary CTA in the vascular morphological diagnosis
