四君子汤合温胆汤加减配合背俞指针疗法治疗脾虚湿热型胃食管反流病的效果

摘　　要：目的探讨四君子汤合温胆汤加减配合背俞指针疗法治疗脾虚湿热型胃食管反流病(GERD)的效果。方法选取2020年7月至2022年1月收治的92例脾虚湿热型GERD患者作为研究对象,按照随机对照原则将其分为对照组和观察组,每组46例。对照组采用常规西药治疗,观察组采用四君子汤合温胆汤加减配合背俞指针疗法治疗。比较两组的治疗效果。结果治疗后,观察组的反酸、口苦或黏、食后腹胀、胃痛、食欲减退、疲乏无力、便溏评分及总分均显著低于对照组(P<0.05)。治疗后,观察组的瘦素(LEP)水平显著低于对照组,生长激素释放肽(Ghrelin)水平及食管上括约肌静息压(UESP)、食管下括约肌静息压(LESP)、远端收缩积分(DCI)高于对照组(P<0.05)。结论四君子汤合温胆汤加减配合背俞指针疗法能够明显降低脾虚湿热型GERD患者的中医证候积分,提升临床疗效,改善食管压力参数情况,调节LEP、Ghrelin水平。Objective To explore the effect of modified Sijunzi decoction and Wendan decoction combined with back-shu pointer therapy in the treatment of gastroesophageal reflux disease(GERD)of spleen deficiency and dampness-heat type.Methods A total of 92 patients with GERD of spleen deficiency and dampness-heat type admitted from July 2020 to January 2022 were selected as the research objects.According to the principle of randomized control,the patients were divided into control group and observation group,with 46 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with modified Sijunzi decoction and Wendan decoction combined with back-shu pointer therapy.The therapeutic effects of the two groups were compared.Results After treatment,the acid reflux,bitter or sticky mouth,abdominal distension after eating,stomach pain,loss of appetite,fatigue,loose stool scores and total score in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the level of leptin(LEP)in the observation group was significantly lower than that in the control group,and the level of growth hormone releasing peptide(Ghrelin),upper esophageal sphincter resting pressure(UESP),lower esophageal sphincter resting pressure(LESP)and distal contractile index(DCI)were higher than those in the control group(P<0.05).Conclusion Modified Sijunzi decoction and Wendan decoction combined with back-shu pointer therapy can significantly reduce the Chinese medicine syndrome scores of patients with GERD of spleen deficiency and dampness-heat type,ameliorate clinical efficacy,improve esophageal pressure parameters,and regulate LEP and Ghrelin levels.
