胰腺浆液性囊腺瘤临床病理分析

摘　　要：目的探讨胰腺浆液性囊腺瘤的临床病理学特征、诊断及治疗。方法对2014年9月—2022年4月经手术切除并经组织学证实的17例胰腺浆液性囊腺瘤的临床资料进行回顾性分析。结果本组17例患者中女性12例,男性5例,年龄30~81岁。其中11例为微囊型浆液性囊腺瘤,6例为寡囊型浆液性囊腺瘤。12例患者无明显临床症状,5例表现上腹部闷痛、体质量减轻等症状。影像学检查CT多表现为低密度影,MR则显示为长T1长T2信号影。影像学检查对于囊腺瘤的诊断正确率为64.7%(11/17),对浆液性囊腺瘤的诊断正确率为52.9%(9/17)。显微镜下肿瘤细胞形态温和,边界清晰,胞浆因富含糖原呈透亮,特殊染色PAS为阳性而D-PAS为阴性。17例浆液性囊腺瘤均行手术治疗,其中3例为胰十二指肠切除,14例为胰腺肿物切除。17例均获随访,随访时间3~5个月,术后皆无复发及转移。结论胰腺浆液性囊腺瘤是一种良性肿瘤,好发于中老年女性,影像学检查对于术前诊断有一定帮助,但最终诊断仍需要病理检查,该病通过外科手术切除一般可治愈。此文的研究成果将用于指导临床医生对这类疾病的诊疗和开展相关科研项目。Objective To explore the clinical pathological characteristics,diagnosis,and treatment of pancreatic serous cystadenoma.Methods A retrospective analysis was conducted on the clinical data of 17 patients with pancreatic serous cystadenoma who underwent surgical resection and histological confirmation from 2014 to 2022.Results Among the 17 patients in this group,there were 12 females and 5 males,with an age range of 30-81 years.Among them,11 cases were microcystic serous cystadenoma,and 6 cases were oligocystic serous cystadenoma.12 patients had no obvious clinical symptoms,and 5 patients showed epigastrium pain and weight loss.Imaging examinations often show lowdensity shadows on CT,while MRI shows long T1 and long T2 signal shadows.The diagnostic accuracy of imaging examination for cystadenoma was 64.7%(11/17),and the diagnostic accuracy for serous cystadenoma was 52.9%(9/17).Under the microscope,the morphology of tumor cells is mild,with clear boundaries.The cytoplasm is transparent due to its abundant glycogen,and special staining for PAS is positive while D-PAS is negative.17 cases of serous cystadenoma underwent surgical treatment,including 3 cases of pancreaticoduodenectomy and 14 cases of pancreatic mass resection.All 17 cases were followed up for 3-5 months,and there was no recurrence or metastasis after surgery.Conclusion Pancreatic serous cystadenoma is a benign tumor that is more common in middle-aged and elderly women.Imaging examination is helpful for preoperative diagnosis,but the final diagnosis still requires pathological examination.This disease can generally be cured through surgical resection.
