摘　　要：目的研究分析HIV-1感染者与健康者接种乙型肝炎疫苗后出现的免疫应答差异。方法将在2021年2月-2023年2月期间到医院接受乙型肝炎疫苗接种的30例HIV-1感染者作为感染组,选择同期接受乙型肝炎疫苗接种的30例健康者作为对照组,于两组接种前、接种后4周和12周分别收集血液样本,对其血液标本采用酶联免疫法进行血清中乙型肝炎表面抗原和抗体水平检测,同时采用流式细胞术对CD4+T细胞的计数和病毒载量进行测定。结果在接种结束后第4周以及第12周时,HIV-1感染者的抗-HBs水平均低于健康者;在接种结束后第4周时,HIV-1感染者呈阳性率低于健康者,比较差异有统计学意义,P<0.05;在接种结束后第12周时,HIV-1感染患者的呈阳性率与健康者对比,差异有统计学意义,P<0.05。两组受试者在接受乙型肝炎疫苗接种后第4周以及第12周的细胞免疫反应对比,产生γ干扰素(IFN-γ)的CD4+T细胞以及CD8+T细胞的百分比对比,HIV-1感染者均比健康者的低,数据差异显著,P<0.05。结论HIV-1感染者接种乙型肝炎疫苗后的免疫应答与健康接种者存在差异,可能与CD4+以及CD8+T细胞计数和病毒载量的降低有关。Objective To Study and analyze the differences in immune response between HIV-1 infected indi⁃viduals and healthy individuals after receiving hepatitis B vaccine.Methods 30 HIV-1 infected individuals who received hepatitis B vaccine at the hospital from February 2021 to February 2023 will be selected as the infection group,and 30 healthy individuals who received hepatitis B vaccine at the same time will be selected as the control group,Blood samples were collected from patients before,4 weeks,and 12 weeks after vaccination,and their serum levels of hepatitis B surface antigen and antibody were measured using enzyme-linked immunosorbent assay.CD4+T cells were counted and viral load was measured using flow cytometry.Results At the 4th and 12th weeks after vaccination,the levels of anti HBs in HIV-1 infected patients were lower than those in healthy individuals,with a comparative statistical value of P<0.05;At the 4th week after vaccination,the positivity rate of HIV-1 infected indi⁃viduals was lower than that of healthy individuals,with a statistically significant difference(P<0.05);At the 12th week after vaccination,there was statistically significant difference in the positivity rate of HIV-1 infected patients compared to healthy individuals,P<0.05.Comparison of cellular immune responses between two groups of subjects after receiving hepatitis B vaccine at the 4th and 12th weeks,resulting inγInterferon(IFN-γ)The percentages of CD4+T cells and CD8+T cells in HIV-1 infected individuals were lower than those in healthy individuals,with sig⁃nificant differences in data(P<0.05).Conclusion There are differences in the immune response of HIV-1 in⁃fected individuals after receiving the hepatitis B vaccine,which may be related to a decrease in CD4+and CD8+T cell counts and viral load.
