恩替卡韦联合益肝清症汤对慢性乙型肝炎患者肝功能及门静脉血流动力学的影响

摘　　要：目的探究恩替卡韦联合益肝清症汤对慢性乙型肝炎(简称“乙肝”)患者肝功能及门静脉血流动力学的影响。方法选取本院2020年7月至2022年7月收治的114例慢性乙肝患者,随机数字表法将其设为对照组(57例)与研究组(57例)。对照组接受常规西药及恩替卡韦治疗,研究组在此基础上联合益肝清症汤治疗。观察2组疗效,并对2组中医证候积分、肝功能、门静脉血流动力学及T细胞亚群指标进行治疗前、治疗6个月后对比。结果研究组治疗总有效率93.0%,比对照组79.0%高,差异有统计学意义(P<0.05)。2组治疗后中医证候积分、丙氨酸转氨酶(ALT)、总胆红素(TBil)、天冬氨酸转氨酶(AST)比治疗前低,且研究组比对照组低,差异有统计学意义(P<0.05)。2组治疗后门静脉血流量(Qpv)、门静脉内径(Dpv)比治疗前低,门静脉血流速度(Vpv)比治疗前高,且研究组优于对照组,差异有统计学意义(P<0.05)。2组治疗后CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)比治疗前高,且研究组比对照组高,差异有统计学意义(P<0.05)。结论恩替卡韦联合益肝清症汤可明显改善慢性乙肝患者肝功能及门静脉血流动力学指标,提高临床疗效。Objective To explore the effect of entecavir combined with Yiganqingzheng Decoction on liver function and portal vein hemodynamics in patients with chronic hepatitis B(hepatitis B).Methods One hundred and fourteen patients with chronic hepatitis B admitted to our hospital from July 2020 to July 2022 were randomly assigned to the control group(n=57)and the study group(n=57).The control group was treated with conventional western medicine and entecavir,and the study group was treated with Yigan Qingzheng Decoction on this basis.Observe the curative effect of the two groups,and compare the TCM syndrome score,liver function,portal vein hemodynamics and T-cell subgroup indexes of the two groups before and after treatment for 6 months.Results The total effective rate of the study group was 93.0%,which was higher than 79.0%of the control group(P<0.05).After treatment,the TCM syndrome score,alanine aminotransferase(ALT),total bilirubin(TBil)and aspartate aminotransferase(AST)of the two groups were lower than those before treatment,and those in the study group were lower than the control group(P<0.05).The portal vein blood flow(Qpv),portal vein internal diameter(Dpv)and portal vein blood flow velocity(Vpv)of the two groups were lower than those before treatment,and the study group was better than the control group(P<0.05).After treatment,CD4^(+),CD8^(+),CD4^(+)/CD8^(+)in the two groups were higher than those before treatment,and those in the study group were higher than the control group(P<0.05).Conclusion Entecavir combined with Yiganqingzheng Decoction can significantly improve the liver function and portal vein hemodynamics in patients with chronic hepatitis B,and improve the clinical efficacy.
